Assumption of Risk: I understand that participating in dance classes at Tiffany & Co Academy Of Dance. involves physical activity and carries inherent risks. I voluntarily assume all risks associated with dance activities, including but not limited to, sprains, strains, falls, and other dance-related injuries.

Health and Medical Condition: I certify that I am physically fit and have no medical conditions that would prevent me from participating in dance classes. If I have any pre-existing medical conditions, I have disclosed them to the dance studio's staff.

Release of Liability: In consideration of being allowed to participate in dance classes at Tiffany & Co Academy Of Dance, I hereby release and discharge Tiffany & Co Academy Of Dance., its owners, instructors, staff, and affiliates from any and all liability for any injury, damage, or loss that may occur during my participation in dance classes or events.

Authorization for Emergency Treatment: I authorize the dance studio's staff to seek medical attention on my behalf in the event of any medical emergency during dance classes or events. I understand that the dance studio will make reasonable efforts to contact emergency services and my designated emergency contact, but they are not responsible for the availability or timeliness of such services.

Photography and Video Release: I grant Tiffany & Co Academy Of Dance. the right to photograph or record video footage of me during dance classes or events for promotional purposes. I understand that my image may be used in print materials, on the dance studio's website, and on social media platforms.

Code of Conduct: I agree to abide by the dance studio's code of conduct, which includes treating fellow participants, instructors, and staff with respect and following the studio's rules and guidelines.

Participation of Minors: If the participant is a minor, I, as the parent or legal guardian, agree to all the terms outlined in this admissions waiver on behalf of the minor.

By signing this admissions waiver, I acknowledge that I have carefully read and fully understand its contents. I voluntarily agree to its terms and conditions, and I am aware of the legal consequences of signing this document.

I agree to the NO Bullying of teachers and Students of any kind. NO Slander, NO provocative commentary on Social Media, NO Belittling or any Deformation of character when it comes to Tiffany & Co Academy of Dance and its students/teachers. I understand breaking this policy will result in immediate termination!

I understand to help with the spread of COVID-19 & Monkey Pox my child will wear a mask inside the studio at all times. I understand that I will be responsible for providing my child with their own sanitizing products such as disinfectant sprays, hand sanitizer & mask. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. I understand if my child(ren) present any symptoms related to colds, viruses & COVID I will not send them into the studio knowingly. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Tiffany & Co Academy Of Dance their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

Recital Expectations for Tiffany & Co Academy Of Dance

We are excited to have every student participate in our upcoming dance recital at Tiffany & Co Academy Of Dance. Our recital is typically the 3rd or 4th Sunday of June each season. To ensure a successful and memorable event for everyone involved, please take note of the following recital expectations:

Attendance and Rehearsals: Regular attendance at all scheduled rehearsals leading up to the recital is essential. Attendance is crucial for learning and perfecting choreography. Attendance will depict the length of a dancers performance if not ready. Please inform your dance instructor in advance if you are unable to attend any rehearsals.

Punctuality: Arrive on time for all class rehearsals and the recital itself. Being punctual ensures a smooth flow of the event and allows adequate time for warm-ups and preparations.

Costume Fittings and Care: Attend all costume fittings as scheduled by the studio. Take proper care of your recital costume to keep it clean and in good condition. Avoid eating or drinking while wearing the costume to prevent stains or damage.

Dress Rehearsal: Participate in the dress rehearsal, as it provides a final run-through of the entire recital program and helps resolve any last-minute concerns.

Cost for tickets & recital fee packages can range from $400-$600 depending on family size and tickets

Behavior and Respect: Show respect and consideration towards fellow dancers, instructors, staff, and volunteers. Positive behavior backstage and during the performance contributes to a supportive and friendly atmosphere.

Backstage Etiquette: Follow backstage guidelines provided by the studio. Maintain a quiet and focused atmosphere backstage to ensure smooth transitions between performances.

Hair and Makeup: Follow the specified hair and makeup guidelines provided by the studio. Proper grooming enhances the overall presentation of the dancers.

Guest Tickets and Seating: Invite family and friends to the recital, but be mindful of the allocated number of guest tickets. Make sure your guests are aware of the recital's date, time, and any venue guidelines.

Professionalism: Embrace the stage with confidence and enthusiasm. Remember that the recital is an opportunity to showcase your hard work and talent.

Enjoyment and Support: Above all, have fun and enjoy the experience! Support your fellow dancers and celebrate their performances with positivity and encouragement.

I further understand that in the event of an emergency or accident, the dance studio will make reasonable efforts to contact emergency services and notify my designated emergency contact. However, I also acknowledge that the dance studio is not responsible for the availability or timeliness of emergency services.

In the case of any medical treatment required during my participation in dance activities at Tiffany & Co. Academy Of Dance, I authorize the dance studio's staff to seek medical attention on my behalf. I agree to assume all responsibility for any medical expenses incurred as a result of any injury sustained during dance classes or events. This includes but not limited to first aid resources or ibuprofen.

I certify that myself, child(ren) are physically fit and have no medical conditions that would prevent me from participating in dance activities. If I have any pre-existing medical conditions, I have informed the dance studio's staff about them.

By signing this emergency waiver, I acknowledge that I have carefully read and fully understand its contents. I voluntarily agree to its terms and conditions, and I assume all risks associated with participating in dance activities at Tiffany & Co. Academy Of Dance

Please note that if you choose to withdraw your child from classes at any point during the season, you will be responsible for paying any and all outstanding balances, including tuition for the current month in which you are withdrawing. As a small business, we operate on a student-to-student basis to cover our overhead costs such as staff, space, and program expenses. Your commitment helps us maintain the quality of training, opportunities, and experiences we provide for every dancer.

I agree to pay the one time annual registration fee of $40 and thereafter the monthly cost of my tuition. Payment Plans: Plan A: Pay by parents Portal between on the 1st of the month ( REQUIRED PAYMENT CREDIT OR DEBIT CARD AUTO PAY) Plan B: Pay by Check between on the 1st of the month Plan C: Pay Cash between on the 1st of the month ( Emergency Only) Any accounts that are past due by the 5th of the month will be subjected to pay a late fee of $15.00 (Please note if monthly tuition is not paid by due date, your child will not be permitted to attend class. All payments are nonrefundable and nontransferable. If you plan to withdraw your child from our dance program a written email and/or letter needs to be submitted 14 days prior to the next months payment. Any payments declined on payment portal or any returned checks made to Tiffany & Co. Academy of Dance The returned check/declined card fee is $35. Should this provision have to be enforced by legal means, the undersigned person(s) is responsible for payment, as liquidated damages, the costs of collection, plus interest at the legal rate and reasonable attorney's fees as determined by the Court or 15% of the amount collected failing such determination. PLEASE NOTE WE DO NOT PROVIDE ANY REFUNDS FOR REGISTRATION, TUITION AND OR COSTUMES!

Ticket Purchase: By purchasing a ticket for the Tiffany & Co Academy Of Dance. recital event, I understand that I am granted admission to the specified performance(s) on the date(s) indicated on the ticket. PLEASE NOTE EVERY SINGLE DANCER WILL BE REQUIRED TO SELL 9 TICKETS AT $35 PER TICKET, 13 TICKETS WILL BE REQUIRED FOR 2 OR MORE DANCERS. PLEASE NOTE WE ALSO HAVE MANDATORY RECITAL FEES AS WELL & parking associated with the building

Assumption of Risk: I voluntarily assume all risks associated with attending the dance recital, including but not limited to, accidents, injuries, property damage, or any other incidents that may occur during the event.

Release of Liability: I release Tiffany & Co Academy Of Dance., its owners, instructors, staff, and affiliates from any and all liability for any injury, damage, loss, or harm to myself or my property that may arise while attending the recital event.

Photography and Video Recording: I understand that photography and video recording may take place during the recital event. By attending, I grant Tiffany & Co Academy Of Dance. permission to use my likeness, image, or voice in promotional materials for the studio without compensation or further consent.

Ticket Refunds and Exchanges: I acknowledge that recital tickets are non-refundable and non-transferable. Once purchased, tickets cannot be exchanged for another date or performance.

Lost or Stolen Tickets: Tiffany & Co Academy Of Dance is not responsible for lost, stolen, or damaged tickets. It is my responsibility to keep my ticket secure and present it for admission to the recital event.

Venue Policies: I agree to abide by the policies and rules of the recital venue, as well as any instructions provided by [Tiffany & Co Academy Of Dance., its staff, or event organizers.

By purchasing and using the recital ticket, I acknowledge that I have carefully read and fully understand its contents. I voluntarily agree to its terms and conditions, and I am aware of the legal consequences of attending the recital event.

Indicated in this space are any health problems or conditions of which the studio should be aware (such as heart, back, medical, allergy, muscular, pregnancy, diabetes, epilepsy, chemical or neurological condition, special medication, knee/kidney/shoulder problems, etc.). I understand that risk of injury is inherent in any physical activity and I, on behalf of myself and my child, knowingly and voluntarily accept that risk. I, the undersigned, for myself, my heirs, administrators, and executors, hereby waive and release Tiffany & Co. Academy Of Dance individually and Tiffany & Co. Academy Of Dance, LLC. and its staff from any and all claims or damages of any kind arising out of my child's participation in the exercise and/or dance program of Tiffany & Co. Academy Of Dance. I further certify that the aforementioned student is in proper physical condition to participate in the exercise/dance program and that he/she has been examined by a licensed physician and found to be in proper physical condition to participate in said program. I, the undersigned, do hereby authorize Tiffany & Co. Academy Of Dance or her designated agents (being Teachers or administrators employed by Tiffany & Co. Academy Of Dance to obtain medical treatment for my said child in emergency situations where I cannot be reached in time to authorize the treating physician to provide such emergency medical services. I understand that I am responsible for any medical expenses and that the absence of health insurance does not make Tiffany & Co. Academy Of Dance responsible for payment of medical expenses. This authority includes the power to authorize any and all treatment deemed necessary under the circumstances by a licensed physician. This power is in essence a power of attorney and shall remain in effect for one year from this date.

I hereby acknowledge and agree to the terms and conditions set forth in this slander waiver for Tiffany & Co Academy Of Dance.

Non-Disparagement: I agree not to make any negative or defamatory statements, either verbally or in writing, about Tiffany & Co Academy Of Dance, its owners, instructors, staff, or affiliates, on any public platform, including but not limited to social media, online forums, and public reviews.

Protection of Reputation: I understand that slanderous or disparaging remarks can harm the reputation and goodwill of Tiffany & Co Academy Of Dance. Therefore, I will refrain from engaging in any conduct that could damage the studio's reputation.

Dispute Resolution: In the event of any disagreement or dissatisfaction with Tiffany & Co Academy Of Dance, its services, or any related matter, I agree to address the issue directly with the studio's management or designated representative through appropriate channels. I will seek to resolve any disputes in a respectful and constructive manner.

Consequences of Violation: I acknowledge that any breach of this slander waiver may result in legal action taken by Tiffany & Co Academy Of Dance to protect its reputation and seek damages for any harm caused by such violation.

Scope of Waiver: This slander waiver applies to all current and future interactions and experiences I may have with Tiffany & Co Academy Of Dance, its owners, instructors, staff, and affiliates.

By signing this slander waiver, I acknowledge that I have carefully read and fully understand its contents. I voluntarily agree to its terms and conditions, and I am aware of the legal consequences of signing this document.

Tuition, Withdrawal, Recital & Competition Policy Agreement

By enrolling my child at Tiffany N Company Academy of Dance (TCAD), I acknowledge that I have read, understand, and agree to the following policies.

Tuition & Withdrawal Policy
Tuition is due according to the payment schedule selected at registration.
A written withdrawal request is required at least fourteen (14) days prior to your next scheduled tuition payment.
If written notice is not received at least fourteen (14) days before the next payment, you will remain financially responsible for that payment and any other charges already incurred.
The 14-day withdrawal policy does not apply during the final sixty (60) days leading up to the annual recital. During this period, recital policies supersede the standard withdrawal policy.
Verbal notice, text messages, emails without confirmation, or simply not attending class do not constitute an official withdrawal.
Enrollment remains active until TCAD receives written notice and all financial obligations have been satisfied.
Withdrawal / Opt-Out Fees
Recreational Program

A minimum Recreational Program Opt-Out Fee is required for any dancer who withdraws from the program.

This fee helps offset:

Reserved class placement
Instructor scheduling
Administrative costs
Choreography already completed
Recital preparation expenses
Your final month's tuition obligation

The Recreational Opt-Out Fee will be equal to your final month's tuition or any outstanding recital-related expenses, whichever is greater.

Competition Team

Competition Team members make a full-season commitment.

Any Competition Team dancer who withdraws after accepting a position on the team will be assessed a:

$250 Opt-Out Fee PER DANCE

This fee is separate from and in addition to any unpaid balances for:

Choreography
Competition entry fees
Costumes
Training fees
Production fees
Studio expenses already incurred on behalf of the dancer

Payment of the opt-out fee does not eliminate responsibility for other outstanding financial obligations.

Recital Commitment

Participation in the annual recital is a commitment for the entire dance season.

If you choose to withdraw your child within sixty (60) days of the recital, you remain financially responsible for all recital-related expenses, including but not limited to:

Costume fees
Costume deposits
Production fees
Recital participation fees
Remaining tuition
Any outstanding recital balances

Because choreography, formations, spacing, music editing, lighting, production planning, and staging are completed well in advance, your child's placement affects every dancer in the performance.

For this reason, families withdrawing during the final sixty (60) days remain financially responsible even if their child no longer participates.

No refunds or credits will be issued for:

Costumes
Recital fees
Production fees
Tuition
Choreography
Any recital-related expenses
Mandatory Dress Rehearsal & Performance Policy

Dress Rehearsal is MANDATORY for every recital participant.

Failure to attend Dress Rehearsal may result in immediate removal from one or all recital routines at the discretion of the Artistic Director.

This policy exists for the safety of every dancer and the quality of the performance.

Even if a dancer is removed from a routine because of an unexcused absence:

All tuition remains due.
All costume fees remain due.
All recital fees remain due.
No refunds or credits will be issued.
Mandatory Rehearsal Policy

Attendance is essential to every group performance.

The following are considered mandatory:

Weekly rehearsals
Competition rehearsals
Boot Camps
Choreography weekends
Intensive rehearsals
Dress Rehearsals
Studio-wide mandatory rehearsals
Any rehearsal designated mandatory by TCAD

Missed rehearsals:

Are not eligible for refunds or make-up credits.
Do not reduce tuition.
Do not reduce recital fees.
Do not reduce costume fees.
May result in removal from dances, formation changes, or reassignment at the Artistic Director's discretion.

Parents understand that every dancer shares responsibility within each routine, and repeated absences impact the entire team.

Costume Policy

Once costume measurements have been submitted, costumes have been ordered, or deposits have been paid:

Costume fees become NON-REFUNDABLE.
Costume fees are NON-TRANSFERABLE.
Costumes cannot be returned due to withdrawal, illness, injury, scheduling conflicts, or change of mind.
Parents remain responsible for all unpaid costume balances.
Competition Team Commitment

Competition Team dancers commit to the full competitive season.

Families understand that choreography, music editing, formations, competition registrations, coaching schedules, and production planning begin months in advance.

Withdrawal from the team does not eliminate financial responsibility for fees already incurred.

The Artistic Director reserves the right to replace dancers or restage routines when necessary.

General No Refund Policy

All payments made to Tiffany N Company Academy of Dance are FINAL.
