1. RELEASE OF LIABILITY
I hereby acknowledge and understand that participating in on-premise classes at:
1040 Montauk Highway, Shirley, NY
may require a certain degree of physical fitness, agility, stamina, and strength, and may involve strenuous physical activity. I voluntarily assume full responsibility for any and all risks, injuries, or damages that may arise as a result of participation in any class or activity offered by Legworks Dance Studio Ltd.
I hereby release, waive, and agree to hold harmless Legworks Dance Studio, its owners, operators, employees, and instructors from any and all liability, including but not limited to:
Personal injury
Property damage
Illness or death
Legal claims, demands, actions, causes of action
Expenses and costs, including attorney’s fees
which arise out of or relate in any way to my (or my child’s) participation in any class, event, or activity at Legworks Dance Studio.
2. MEDICAL EMERGENCIES
As the legal parent or guardian, I give permission to Legworks Dance Studio, its owners, and operators to seek medical treatment for my child(ren) in the event they are unable to reach me.
I declare the participant(s) to be in good physical and mental health.

3. PHOTO / MEDIA RELEASE
I give permission to Legworks Dance Studio to photograph or video record myself and/or my child(ren) while participating in any studio activities, including classes, events, and performances. These images may be used for promotional, social media, and marketing purposes.
4. MASK POLICY
Masks are optional while participating in any in-studio activity or class.
5. COVID-19 RELEASE
As I or my child(ren) enroll to participate in on-premise classes at Legworks Dance Studio beginning September 14, 2026, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I or my child(ren) may be exposed to or infected by COVID-19. I understand that such exposure could result in:
Illness or injury
Permanent disability
Death
By signing this agreement, I hereby release, discharge, covenant not to sue, and hold harmless Legworks Dance Studio, its employees, agents, and representatives from any and all claims, including but not limited to:
Illness
Injury
Death
Property damage
Legal actions or costs
arising from exposure to COVID-19 related to participation in studio programs, whether caused by negligence or otherwise. This release applies regardless of whether exposure occurs before, during, or after participation in any studio activity.
AGREEMENT & SIGNATURE
I have read and fully understand the above Release of Liability, Medical Consent, Photo/Media Release, Mask Policy, and COVID-19 Waiver. I voluntarily agree to all terms.
