I have read and understand the Vision Athletics Rules, regulations and agreements. I know that Vision Athletics is not responsible for any injuries while on premises. I understand that I should not bring my child if they have had a fever within 24 hours and have the option to makeup classes with a scheduled private lesson if I feel necessary. I agree to bring my athlete dressed properly and ready for class each week.

I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury and, in rare circumstances, death. I agree to release and hold harmless Vision Dance and Cheer Ministry, including its teachers, dancers, staff members, and facilities from any cause of action, claims, or demands now and in the future. I will not hold Vision Dance and Cheer liable for any personal injury or any personal property damage which may occur on the premises before, during or after classes. Furthermore, I understand that I should be aware of my, and/or my child's physical limitations and agree not to exceed them. If I am signing this waiver for my child, I certify that I am the parent or legal guardian and have the right to waive these rights.

There is a $40 non-refundable registration fee per student, due upon enrollment for the School Year. Tuition is due on the 1st of each month and is non-refundable and non-transferable. Dance shoes will be ordered at the beginning of the season and will be between $20 and $60 and available in Starstruck dancewear. An additional costume fee of $100 will be added for each class participating in the recital in November. There will also be a performance fee of $40 due in January for dancers participating in the End-of-Year Recital. These are the only additional payments. Payments should primarily be made on the Dance studio pro parent portal. You can also pay by check or cash. If paying with check or cash, be sure to get a receipt. Make checks payable to Vision Dance, and be sure to write your child's name in the memo line. There is a $30 fee for returned checks. Tuition that is not paid by the 10th of the month will be subject to a $25 late fee. All other fees need to be paid as soon as possible. Any additional fees not paid after 30 days will be subject to a $40 late fee.

I hereby grant the Vision Athletics Ministry permission to use my photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.

I understand and agree that all photos will become the property of the Vision Athletics Ministry and will not be returned.

I hereby irrevocably authorize the Vision Athletics to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I hereby hold harmless, release, and forever discharge the Vision Athletics Ministries from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
