I, the undersigned parent/legal guardian, give permission for _____________________ (student’s name) to attend and participate in dance classes, rehearsals, performances, competitions, camps, and other activities offered by All In Dance Company.
I understand that participation in dance and related activities involves physical activity and carries an inherent risk of illness, injury, or accident. By signing this waiver, I voluntarily assume all risks associated with participation.
On behalf of myself, my child, my heirs, executors, and anyone who may claim on our behalf, I hereby waive, release, and forever discharge All In Dance Company, its owners, instructors, staff, volunteers, and representatives from any and all claims, liabilities, demands, damages, or causes of action arising out of or related to any injury, illness, loss, or damage that may occur while participating in or attending any All In Dance Company activity, except where prohibited by law.
I also understand and agree that any photographs, videos, or other media taken during classes, rehearsals, performances, competitions, camps, or other studio events are the property of All In Dance Company and may be used for promotional, marketing, social media, website, or educational purposes without compensation.
By signing below, I acknowledge that I have read, understand, and agree to the terms of this Liability Waiver, Photo Release, and Emergency Medical Authorization.
