PHOTOGRAPHY & LIABILITY RELEASE and WAIVER

Sarasota Contemporary Dance, Inc.

I hereby grant Sarasota Contemporary Dance, its representatives, agents, and employees the right to take photographs and video recordings of me [while participating in SCD Classes, Intensives,Camps and Events. I authorize Sarasota Contemporary Dance, its transferees, and assigns to copyright, use, and publish photographs and video recordings of me in any medium, including print and digital, without or without using my name, for any lawful purpose (including, for example, printed promotional materials or on a web site).

I hereby release, waive, and discharge Sarasota Contemporary Dance, its officers, directors, representatives, agents, employees, successors, transferees, and assigns from any and all liabilities and claims, known or unknown, contingent or otherwise, related to or resulting from the use of my image or likeness.

I also understand that I am completely responsible for taking care of my own physical and emotional well being during the SCD Programs. I understand that Sarasota Contemporary Dance, its representatives who are conducting the program, and its designees and facilities shall not be liable for any personal injuries sustained by myself throughout the program presented by Sarasota Contemporary Dance.

Participant warrants and represents that he/she is authorized to sign this Release and Waiver of Liability and expressly assumes the full risk of personal injury and responsibility for any and all injuries or damages that may occur during the Program.

By signing this agreement, I acknowledge that I have read, understand, and agree to the terms and conditions of this Release and Waiver of Liability.
