As the legal parent or guardian, I release and hold harmless Cartersville School of Ballet, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or in route to or from any of said premises. I understand that appropriate physical contact is required during the instruction of dance and I give permission for instructors to make appropriate physical contact with me or my child for such instruction.
I understand that I am registering for the nine-month season from September 2025 through May 2026. If I must withdraw after August 29, 2025 one month’s written notice is required to be sent to our email address. A student is not considered withdrawn until the written notice is received by the office manager. Parent/responsible party will continue to be billed without written notice of withdrawal. I understand that no refunds are given for classes missed but make-up classes may be taken within a month of classes missed. The following fees are non refundable: Registration and Costume Fees. I understand that a Costume/Recital Fee is due by January 15, 2026. Costume fees are non-refundable. The office must be given written notice no later than January 2, 2026 of any student not planning to perform in the end of the year student concert. Costume Fees are $85 per student per class.
I give permission for Cartersville School of Ballet to take photos and or videos of classes, rehearsals and/or performances, including dancers of Cartersville School of Ballet in dancewear/costumes, me or my child while participating in Cartersville School of Ballet activities. Names of students will not be used or disclosed. I understand that I am giving permission to use these photos and/or videos for publishing use, in printed or electronic format.
This agreement outlines the commitment of parents and guardians to ensure the health and safety of all students attending dance classes at Cartersville School of Ballet. By signing this agreement, parents and guardians affirm their understanding of and adherence to the following guidelines. Health and Safety Commitment To maintain a healthy environment for all students and staff, parents and guardians agree to the following: Monitoring Health: Parents and guardians will actively monitor their child's health and ensure that they are free from contagious illnesses before attending dance class. Symptoms of Illness: Parents and guardians agree not to send their child to dance class if they exhibit any of the following symptoms: Fever or chills, Cough or sore throat, Shortness of breath or difficulty breathing, Fatigue or muscle aches, Loss of taste or smell, Nausea, vomiting, or diarrhea Communication: If a child is diagnosed with a contagious illness or has been in close contact with someone who is, parents and guardians agree to promptly inform the dance school administration. Return to Class: Children who have been ill may return to class only after they have been symptom-free for at least 24 hours without the aid of medication, or as guided by a healthcare professional. Responsibility and Cooperation By signing this agreement, parents and guardians acknowledge their responsibility in contributing to a safe and healthy learning environment. Cooperation and honest communication are essential to protect all students and staff members.
