MEDICAL EMERGENCIES
The undersigned gives permission to Dance Innovations Inc., its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health. I request that our doctor/physician be called and that my child be transported to the hospital.
PAYMENT POLICY
I understand that by registering with Dance Innovations Inc., I am responsible for submitting all fees within 7 days. Fees are payable in two convenient methods and can be selected as the "membership type" during the registration process:
1) Monthly - make nine payments dated the first of every month from September 1, 2024 – May 1, 2025
2) Three Month - make three equalized payments, dated September 1, 2024, December 1, 2024 and March 1, 2025.
3) Yearly - make one lump sum payment dated September 1, 2024.
A $35 +GST non-refundable registration fee per child is due at the time of registration along with class fees. A costume deposit of $60 +GST per class is also due with all registration forms to be paid by October 1, 2024. This amount will be deducted from your total costume balance that will be due February 15.
I understand that failure to pay any balance by the tenth day it is due will result in the balance being charged on my credit card.
Payments can be made via e-transfer or credit card. A credit card must be on file for all accounts, even if paying by e-transfer.
I understand that there will be a 15% interest charge on any outstanding fees past 30 days. If fees are not paid within 60 days, clients will be asked to discontinue classes.
I understand that if at any point I would like to change my payment type, it is my sole responsibility to contact Dance Innovations ten (10) days prior to the first of the month.
In the event that you wish to discontinue classes at Dance Innovations, written notice must be received by the studio 15 days prior to the end of the month. For example, if you terminate classes November 14, you will not be required to pay your December fees. If you terminate classes November 20, you will be required to pay for the end of November as well as December. All remaining fees will then be returned / refunded beginning the following month.
RELEASE OF LIABILITY
As the legal parent or guardian, I release and hold harmless Dance Innovations Inc., its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Dance Innovations, its owners and operators or in route to or from any of said premises.
STUDIO BEHAVIOUR
I understand that if any disrespectful behaviour is shown by myself or my family towards staff, students or other parents, that I can be asked to leave the studio immediately with no refund.
SIGNATURE TEXT
I, the undersigned, on behalf of the parties registered, authorize Dance Innovations Inc., use of any photographs and/or videos as well as printing of names for advertising, promotional, and publicity purposes.
PERFORMING ARTS LITTLES (PALs)

SESSIONALS & CAMPS
I understand that by registering for a sessional or camp with Dance Innovations Inc., I am responsible for submitting all fees within 7 days. Payments can be made via e-transfer or credit card. A credit card must be on file for all accounts, even if paying by e-transfer. If payment is not received via e-transfer on the payment due date, the credit card on file will be charged instead.
I understand that there will be a 15% interest charge on any outstanding fees past 30 days. If fees are not paid within 60 days, clients will be asked to discontinue classes.
In the event that you wish to discontinue your enrolment in a sessional class at Dance Innovations, the following refund policies will apply:
1) Refund minus $25 Admin Fee - cancellation is received before the 1st class
2) Prorated Refund - cancellation is received after the 1st class
3) No Refund - cancellation is received after the 2nd class
