Liability Release and Assumption of Risk Waiver
Photography/Video Release

I/We (the dancer and parent/guardian) are aware of the risks associated and agree not to hold or bring any legal action against the company (Ignite Dance Academy Inc.), instructors, employees and facilities used.

I/We will not hold Ignite Dance Academy Inc. liable for any personal injury including but not limited to: scrapes, bruises, cuts, sprains, fractures, broken bones, concussions or death or any personal property damage/loss, which may occur on the premises before, during or after classes.

Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behaviour in addition to any damage I/we may cause to the facilities utilized by Ignite Dance Academy Inc.

Photography/Video - I give permission for the participant/dancer’s photographs/videos to be used for publicity and advertising in but not limited to the Ignite Dance Academy Inc.’s website, social media, and in the community.

By accepting I agree to release the company (Ignite Dance Academy Inc.), instructors, employees and facilities used from any financial responsibility and/or legal actions regardless of the reasons or circumstances.
