I give permission to Diva Dance School to use my name and/or photo in local newspapers, studio brochures, newsletters and advertisements, as well as on websites/social media, etc.
As a registered student/participant of Diva Dance School, I agree to the following waiver and release: I certify that I am “somewhat” physically fit and have not been advised to not participate in the activity or activities requiring a similar level of physical fitness. I certify that there are no health-related reasons or problems which preclude my participation in the activity. I understand there is an inherent risk of accident or injury when participating in a dance or exercise program. I understand there is also a risk of exposure to viruses or other illnesses when you are in contact with other people. I acknowledge that if I am concerned about contracting any illnesses (including COVID-19), I will wear a mask at the studio to protect myself and others. The Diva Dance School name, crown logos and all affiliated copies and designs are the property of Diva Dance School and are not to be used in any print or online form without express written permission from the owners. The school name and logo also may not be used on personally-made merchandise. By signing you are indicating that you recognize and acknowledge all of this and that you waive, release and hold harmless associates of Diva Dance School and their heirs, assigns, employees and independent contractors from any liability for any injury, damage or claim arising from participation in any program of dance instruction or exercise. I have read and understood this agreement and I am aware that by signing this agreement I am waiving certain legal rights which I, or my heirs, executors and representatives may have against Diva Dance School and their heirs, assigns, employees and independent contractors.
