WAIVER, RELEASE & PAYMENT AGREEMENT

I understand and agree that by participating in any dance class, workshop, rehearsal, or performance, there is a possibility of physical injury and, in rare circumstances, death. I voluntarily assume all risks associated with participation.

I agree to release and hold harmless Black Box Studios Miami LLC, including its owners, staff, instructors, dancers, and facility, from any and all causes of action, claims, or demands, now or in the future. I will not hold Black Box Studios Miami LLC liable for any personal injury or personal property damage that may occur on the premises before, during, or after classes.

I understand that I am responsible for being aware of my own and/or my child’s physical limitations and agree not to exceed them. If I am signing this waiver on behalf of my child, I certify that I am the parent or legal guardian and have the legal authority to waive these rights on their behalf.

I further authorize and agree that Black Box Studios Miami LLC may take and use photographs and/or videos of me and/or my child for purposes including, but not limited to, record keeping, advertising, marketing, and promotional use. I understand that I have no ownership rights to these photographs or videos and will not receive compensation for their use.


Tuition & Payment Policy
I understand and agree that all tuition and membership payments are due on or before the 1st of each month. Payments not received by the 5th of the month will incur a late fee of $25.

All membership cancellations or pauses must be submitted in writing via email before the 1st of the month. Requests made after the 1st will apply to the following billing cycle.

I acknowledge that all payments are non-refundable. No refunds or credits will be issued for tuition, memberships, missed classes, or unused time, regardless of attendance.

By signing below, I acknowledge that I have read, understand, and agree to all terms outlined in this waiver and payment policy.
