I understand that participation in dance classes, rehearsals, performances, and related activities involves physical activity and inherent risks, including but not limited to falls, collisions, muscle strains, sprains, fractures, and other injuries.

I confirm that my child is physically able to participate in dance activities and will follow the instructions and safety rules provided by the instructors and staff.

I understand that injuries may occur even when appropriate safety precautions are followed. By signing this waiver, I voluntarily assume the risks associated with my child’s participation in dance activities and agree to release and hold harmless Suzanne's Dance Connection, its owners, instructors, employees, and agents from claims arising from my child’s participation, to the fullest extent permitted by law.

In the event of an injury or medical emergency, I authorize Suzanne's Dance Connection and its staff to obtain reasonable emergency medical care for my child if I cannot be reached promptly. I understand that I am responsible for any resulting medical expenses.

I have read and understand this waiver and agree to its terms. I am the parent or legal guardian of the dancer named above and am signing this agreement voluntarily.
