I do hereby grant permission for my son/daughter to participate in Devotions Dance Co. In order that my child may receive the proper medical treatment in the event that he/she may sustain injury or illness during dance classes, I hereby authorize the staff to obtain or provide medical treatment for my child for such injury or illness. I hereby hold the teacher and sponsoring organizations harmless in the exercise of this authority. I further understand that there is always a possibility that my child may sustain physical illness or injury while attending class. If this occurs, I hereby authorize the staff and representatives to refer my child to a medical treatment center. I further acknowledge and understand that I will be responsible for any medical bills that may be incurred on behalf of my son/daughter for physical illness or injury that he/she may sustain. Understanding that there is always a possibility that my child may sustain physical illness or injury, I acknowledge and understand that my child is assuming the risk of such physical illness or injury by his/her participation, and I further release the sponsoring organization and its representatives from any claims for personal illness or injury that my child may sustain. In addition I release any photo/video of my child that Devotions Dance Company or staff members may use for advertisement purposes.
I understand the studio reserves the right to cancel a class without refund or makeup class offered for unforeseen weather or emergencies. DDC will always send an email and post online if this is to happen.
I understand it is vital my dancer is in attendance. If more than 2 unexcused classes have been missed before a performance, teachers and staff reserve the right to dismiss dancer from performance with no refund of any kind.
I understand DDC runs Auto-Pay at the end of the month for the upcoming month of tuition.
I understand a late fee of $50 will be added to the account by the 5th and the 15th if tuition is unpaid due to card decline. And the full balance must be paid for continued participation in DDC.
a 30 day notice must be submitted to info@devotionsdancecompany.com for disenrollment.
I understand Devotions Dance Company is a zero refund policy studio.
I understand there will be a $75 performance fee for the summer recital and winter show. I understand this must be paid for participation in the show.
I understand that there will be a costume fee associated with each show.
I understand DDC reserves the right to place all students in their proper class level and to move students to a lower level for students benefit. I Understand fundraisers done within the studio have no cash value and will be use for studio related fees only such as costume fee, performance fee, or tuition.
I understand I am to uphold a positive and respectable environment. If I or my child is unable to do so DDC reserves the right to dismiss the student/family from the studio without refund of any kind.
I understand monthly tuition at Devotions Dance Company is based on a flat monthly rate, not a per-class or weekly fee. I understand due to a missed class on my end I am permitted to reach out to DDC to schedule a makeup class in a similar age/style class.
I understand and will follow the students dress code and policies.
