2026–2027 REGISTRATION WAIVER & POLICY ACKNOWLEDGMENT

By completing registration for the 2026–2027 school year, the registered student and/or the student’s parent or legal guardian acknowledges and agrees to the following policies of Ballet & Dance of Upstate NY.

For purposes of this agreement, “registered student” refers to either an adult student registering for themselves or a minor student being registered by a parent or legal guardian.

TUITION & PAYMENT POLICIES

I understand that registration is for the full 2026–2027 school year and tuition is divided into four quarterly installments.

The 2026–2027 tuition installment dates are:

First Installment: September 2, 2026

Second Installment: October 26, 2026

Third Installment: January 11, 2027

Fourth Installment: March 15, 2027

I understand and agree that a $25 non-refundable registration fee is due at the time of registration.

Tuition may be paid online through the Dance Studio Pro Parent Portal or by check.

If paying by check, payment must be received by Ballet & Dance of Upstate NY prior to the applicable tuition due date.

A $10 per week late fee will be charged for payments received after the due date unless prior arrangements have been made with the school.

A $35 fee will be charged for returned checks.

Costume fees for the annual Student Performance are billed separately and are not included in tuition.

There are no refunds or credits for missed classes.

Tuition will not be prorated for classes already attended.

Payments should not be given directly to instructors.

DANCE STUDIO PRO PARENT PORTAL

I understand that the Dance Studio Pro Parent Portal is the primary location for tuition and account information.

I understand that it is my responsibility to regularly review the Parent Portal for current tuition charges, account balances, payment due dates, payment history, outstanding balances, and any additional fees or charges applied to my account.

I understand that I am responsible for knowing the balance due on my account and ensuring that all tuition and applicable fees are paid by the required deadlines.

I understand that failure to review the Parent Portal does not eliminate responsibility for tuition, late fees, or other charges due on the account.

I am responsible for keeping my payment information and account information current.

ENROLLMENT & WITHDRAWAL

I understand that completing registration enrolls the registered student for the entire 2026–2027 school year.

Withdrawal from classes is permitted only with a signed doctor’s note.

If a student withdraws without a doctor’s note, all fees for the current quarter remain due.

I understand that tuition will not be prorated for classes already attended and that registration fees and other applicable fees are non-refundable.

ATTENDANCE

I understand that consistent attendance is essential to a student’s training and progress.

If a student misses four or more classes in a quarter without making them up, the student may be placed in a lower level until all required make-up classes are completed.

I understand that if a student misses four or more classes during the third or fourth quarter, the student may not be permitted to participate in the annual Student Performance unless specifically authorized by Ballet & Dance of Upstate NY.

There are no refunds or tuition credits for missed classes.

CLASS PLACEMENT & REQUIRED BALLET TRAINING

I understand that students must register for all required ballet classes associated with their assigned level in order to meet Ballet & Dance of Upstate NY training requirements and participate in the annual Student Performance.

The minimum weekly ballet requirements are:

Creative Movement, Pre-Ballet, Ballet I and Ballet II: 1 ballet class per week

Ballet III: 2 ballet classes per week

Ballet IV: 3 ballet classes per week

Ballet V and Ballet VI: 4 ballet classes per week

Ballet VII: 4 ballet classes per week plus required Pointe training when applicable

I understand that if multiple class sections are required for the registered student’s level, the student must be registered for all required classes, not simply one class within that level.

Class placement is determined by Ballet & Dance of Upstate NY faculty and may be based on age, technique, ability, progress, attendance, maturity, safety, and overall training needs.

I understand that placement may be adjusted by faculty when necessary.

EXTRACURRICULAR CLASSES

I understand that students registering for extracurricular dance classes, including Tap, Jazz, Contemporary, Modern, Hip Hop, and similar classes, must also be enrolled in at least one ballet class.

I understand that additional placement or prerequisite requirements may apply to certain classes.

STUDENT PERFORMANCE ELIGIBILITY

I understand that participation in the annual Student Performance is dependent upon meeting Ballet & Dance of Upstate NY’s training, attendance, rehearsal, and performance requirements.

Students must be enrolled in and attending all required ballet classes for their assigned level in order to perform with that level.

I understand that additional rehearsal schedules, costume information, performance requirements, and deadlines may be communicated throughout the school year.

I understand that cell phones, texting, photography, and recording equipment are prohibited during performances and that individuals who do not follow this policy may be asked to leave the theater.

STUDIO CALENDAR

I understand that the Ballet & Dance of Upstate NY website calendar is the primary calendar for the school year.

The calendar includes important dates such as holidays, scheduled school closings, observation weeks, picture days, rehearsals, performances, special events, and other important school information.

I acknowledge that I know where to find the Ballet & Dance of Upstate NY calendar:

I understand that it is my responsibility to review the calendar regularly throughout the school year.

I understand that dates, events, rehearsals, and other information may be added or updated throughout the season and that I am responsible for staying informed of those changes.

STUDIO COMMUNICATIONS

I understand that Ballet & Dance of Upstate NY communicates important information through email, text message, the Parent Portal, the studio website, and/or social media.

I am responsible for making sure that my current email address and cell phone number are on file with Ballet & Dance of Upstate NY and are kept up to date throughout the school year.

I understand that email and text messages may be used for important and time-sensitive communications, including schedule changes, school closings, weather-related cancellations, rehearsals, performances, tuition reminders, registration information, and other important studio updates.

I understand that it is my responsibility to regularly review studio communications and to update my contact information immediately if my email address or phone number changes.

SCHOOL CLOSINGS, WEATHER & CANCELLATIONS

I understand that Ballet & Dance of Upstate NY may close or cancel classes due to inclement weather, emergencies, unsafe travel conditions, building issues, or other circumstances outside the school’s control.

School closing and cancellation notices may be sent by email and/or text message and may also be posted through the Parent Portal, studio website, or social media.

I understand that I am responsible for ensuring Ballet & Dance of Upstate NY has a valid, current email address and cell phone number capable of receiving text messages so that I can receive timely notifications.

I understand that it is my responsibility to check studio communications when weather or other conditions may affect regularly scheduled classes.

I also understand that the Ballet & Dance of Upstate NY website calendar lists scheduled school closings, holidays, and other planned dates when classes are not held.

STUDENT CONDUCT & BULLYING

I understand that Ballet & Dance of Upstate NY is committed to providing a safe, positive, and respectful environment for students, families, faculty, staff, and volunteers.

Bullying, harassment, intimidation, threatening behavior, disruptive behavior, or other inappropriate conduct will not be tolerated.

This includes behavior occurring in the studio, at rehearsals or performances, at Ballet & Dance of Upstate NY events, online, through text messages, or on social media when that behavior affects members of the Ballet & Dance of Upstate NY community.

Students are expected to treat fellow dancers, faculty, staff, volunteers, and families with respect.

I understand that violations of studio conduct policies may result in disciplinary action, removal from an activity or performance, or dismissal from Ballet & Dance of Upstate NY.

ASSUMPTION OF RISK

I understand that participation in dance, conditioning, stretching, rehearsals, performances, and other physical activities involves inherent risks.

These risks may include, but are not limited to, falls, strains, sprains, collisions, muscle injuries, and other physical injuries.

I voluntarily assume the ordinary risks associated with participation in classes, rehearsals, performances, workshops, and other Ballet & Dance of Upstate NY activities.

To the fullest extent permitted by law, I release Ballet & Dance of Upstate NY, its owners, instructors, employees, contractors, volunteers, and affiliated organizations from claims arising from the ordinary risks inherent in participation in these activities.

EMERGENCY MEDICAL AUTHORIZATION

In the event of an emergency and if I cannot be reached, I authorize Ballet & Dance of Upstate NY staff to obtain or arrange reasonable emergency medical care for the registered student.

I understand that I am financially responsible for any medical expenses incurred.

I am responsible for notifying Ballet & Dance of Upstate NY of any medical conditions, allergies, injuries, medications, physical limitations, or other information that may affect the registered student’s safe participation.

PHOTOGRAPHY & VIDEO

I understand that photographs and video may occasionally be taken during classes, rehearsals, performances, and Ballet & Dance of Upstate NY events.

These images may be used for educational purposes, the studio website, social media, marketing, advertising, print materials, or other Ballet & Dance of Upstate NY promotional purposes.

I understand that any separate photo or video permission selection provided during registration will determine whether identifiable photographs or video of the registered student may be used for promotional purposes.

PARENT, GUARDIAN & ADULT STUDENT RESPONSIBILITY

I understand that it is my responsibility to review the registered student’s placement information before registering.

I understand that it is my responsibility to register the student for all required classes for their assigned level.

I understand that it is my responsibility to review and follow Ballet & Dance of Upstate NY tuition and payment policies.

I understand that it is my responsibility to regularly review the Dance Studio Pro Parent Portal for tuition charges, balances, payment history, and other account information.

I understand that it is my responsibility to pay tuition and applicable fees by the stated deadlines.

I understand that it is my responsibility to regularly review the Ballet & Dance of Upstate NY website calendar.

I understand that it is my responsibility to maintain a current email address and cell phone number on file.

I understand that it is my responsibility to read studio emails, text messages, Parent Portal notices, and other communications.

I understand that it is my responsibility to ensure the registered student meets attendance, rehearsal, and performance requirements when participating in the annual Student Performance.

I understand that it is my responsibility to contact Ballet & Dance of Upstate NY with questions regarding placement, tuition, scheduling, or school policies.

FINAL ACKNOWLEDGMENT

By electronically accepting this agreement and completing registration, I acknowledge that I have read, understand, and agree to the Ballet & Dance of Upstate NY policies for the 2026–2027 school year.

I understand that registration is for the full school year.

I understand that tuition is divided into four installments with specific payment due dates.

I understand that tuition and account information is available through the Dance Studio Pro Parent Portal and that it is my responsibility to review the portal regularly.

I understand that check payments must be received prior to the applicable tuition due date.

I understand that late and returned-payment fees may apply.

I understand that withdrawal and refund policies apply once registration is completed.

I understand that attendance may affect class placement and Student Performance eligibility.

I understand that students must register for all required classes associated with their assigned ballet level.

I understand that extracurricular dance classes require enrollment in ballet.

I understand that the Ballet & Dance of Upstate NY website calendar is the primary school calendar, I know where to find it, and it is my responsibility to review it regularly.

I understand that the studio may communicate school closings, weather cancellations, schedule changes, and other important information by email and/or text message.

I understand that it is my responsibility to maintain a current email address and cell phone number capable of receiving text messages so I can receive important studio notifications.

I understand that I am responsible for reviewing studio communications throughout the school year.

I understand that studio schedules, dates, and policies may be updated when necessary.

By electronically accepting this agreement, I understand that my electronic acknowledgment serves as my agreement to these policies.
