Liability Waiver and Acknowledgment of Risk

Synergy Dance Academy, LLC, an Ohio limited liability company, (hereinafter referred to as SDA) will make all reasonable efforts to ensure our students avoid injury. However, it is important to understand and agree that by participating in any dance class, workshop, rehearsal, or performance, there is a possibility and/or risk of serious physical injury, disability, and/or death.

PLEASE READ CAREFULLY AND ACCEPT THE TERMS BELOW. REGISTRATION IS INCOMPLETE WITHOUT ACCEPTANCE AND MUST BE COMPLETED BEFORE YOUR FIRST CLASS.

I hereby acknowledge and agree to be bound by all the terms set forth herein in this Liability Waiver and Acknowledgement of Risk.

I voluntarily agree to assume all risks and responsibilities for any injuries and/or accidents, which could occur to me or my child during any of SDA's classes, rehearsals, performances, and/or activities.

I also exempt, release, and indemnify SDA, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or other students from any and all liability claims, demands, and/or causes of action whatsoever relating to any damage, loss, injury, disability, or death to me or my child, and/or personal property which may arise out of or in connection with participation in any classes, rehearsals, and/or activities conducted by SDA.

I further voluntarily agree to waive my rights and the rights of my heirs, successors, and assigns to hold SDA, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, disability, or death.

I understand that I alone should be aware of my child's medical and/or physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the legal right and/or standing to waive these rights on his/her behalf.

My acceptance is proof of my intention to execute a complete and unconditional waiver all liability, either on behalf of my minor child or on my own behalf, to the fullest extent of the law. I further understand that this is a legally binding contract enforceable under the Laws of the State of Ohio.

I have read, understood and agree to be bound by the above statements.

Synergy Dance Academy has my permission to use photos and/or videos of my child for their social media platforms to include but not limited to their website, FaceBook, and Instagram pages.
