I give my child, permission to participate in the Fine Arts School of Dance program. She/He is physically fit to participate and understands the rules of safety must be followed. Any student, who does not follow the rules, will be withdrawn from class. Understanding that any physical activity program involves an element of risk, I hereby release Fine Arts School of Dance and Christine Butler individually from responsibility for any injury to my child while participating in this class.
I understand that because of the novel coronavirus, COVID-19, Fine Arts School of Dance has put into place additional precautions to maintain a safe a healthy environment. I acknowledge the contagious nature of covid-19 and voluntarily assume the risk that my child and I may be exposed to or infected be covid-19 by attending the studio and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by covid-19 at the studio may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, volunteers, and program participants and their families. I voluntarily agree to assume all the foregoing risks and accept sole responsibility for any injury to my child or myself.
