1. Assumption of Risk
I, the undersigned, recognize and understand that participation in dance classes, rehearsals, workshops, events, performances, community activities, and any related programs (“Activities”) at or sponsored by Miriam’s Dance Academy involves physical movement that may result in injury, illness, or, in rare cases, death. I knowingly and voluntarily assume all risks associated with participation, including but not limited to falls, collisions, muscle strains, sprains, fractures, unforeseen medical conditions, and accidents while on the premises, at off-site events, or while traveling to/from activities.
2. Release of Liability
In consideration of being allowed to participate in Activities, I, on behalf of myself and/or my child, hereby release, waive, discharge, and covenant not to sue H.U.U.R.T. Community Development Corporation dba Miriam’s Dance Academy, its directors, officers, employees, volunteers, agents, landlords, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained, whether caused by negligence or otherwise, while participating in Activities, on or off studio premises, or during related events.
3. Medical Consent
I affirm that I/my child am physically fit to participate in dance activities. In the event of a medical emergency, I authorize Miriam’s Dance Academy staff to obtain medical treatment for myself/my child and agree to be financially responsible for any costs incurred.
4. Photo/Video Consent
I understand that Miriam’s Dance Academy may photograph and/or record classes, performances, and events for promotional, training, and archival purposes. By signing this waiver, I grant permission for my/my child’s image, likeness, and voice to be used in print, video, online, and social media formats without compensation.
Opt-Out Option: If I choose not to consent, I must select the “Photo/Video Opt-Out” option in the Dance Portal. Otherwise, consent is assumed.
5. Special Needs Policy
At Miriam’s Dance Academy, we embrace every child with love and care. However, our staff is not fully trained to work with children who have high special needs. We will always do our best to serve all families by offering a free trial class—with a parent or guardian present—to help determine if additional support is needed.
We handle these situations on a case-by-case basis, working closely with parents/guardians to find the best possible solution for each child. In some cases, we may kindly ask that a parent or guardian remain in class with their child if we feel it is necessary to ensure their safety and success.
6. We have a zero-tolerance policy for physical aggression and harmful behavior.
The following behaviors are strictly prohibited:
Fighting or attempting to fight
Hitting, punching, kicking, or pushing another person
Physical intimidation or threats
Bullying, harassment, or aggressive behavior
Any unwanted physical contact intended to harm, intimidate, or injure another person
Encouraging, participating in, or instigating physical altercations
Any student who engages in fighting, physical aggression, or intentionally puts their hands on another student, staff member, volunteer, parent, or visitor in a harmful or threatening manner will be subject to immediate dismissal from the Academy.
Depending on the severity of the incident, dismissal may occur without warning, refund, or opportunity to return. The Academy reserves the right to determine appropriate disciplinary action and to take any additional measures necessary to protect the safety of its students and staff.
Parents and guardians acknowledge and agree to support this policy as a condition of enrollment.
7. Acknowledgment
I have read, understand, and agree to abide by all policies, rules, and expectations of Miriam’s Dance Academy. By signing this waiver, I agree to all terms and conditions listed above, and I voluntarily release and hold harmless Miriam’s Dance Academy and its affiliates from all liability as stated herein.
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
