Classroom & Student Behavior

I understand that students must consistently meet the following age-appropriate expectations to participate in group classes at South Sound Dance:

1. Follow basic, age-appropriate instructions.
2. Follow classroom safety expectations, including staying in designated areas and using equipment and materials appropriately.
3. Respect instructors and fellow students without excessive disruption, unsafe behavior, or unwanted physical contact.

Students are expected to follow the dress code and footwear requirements for their specific class or program.

Classroom entry, dismissal, and observation procedures vary by program. Families agree to follow instructions provided by SSD staff. Parent observation may be available via Zoom for designated classes.

South Sound Dance is committed to a supportive and respectful environment for all. Bullying, offensive language, harassment, or disrespectful behavior toward students, families, instructors, or staff will not be tolerated.

By checking this box, I acknowledge and accept that participation in dance classes, fine arts preschool, performances, rehearsals, competitions, events, field trips, conventions, and other programs or activities offered or organized by South Sound Dance LLC, whether held on or off Studio/Company premises, may involve inherent risks, including possible injury, illness, or property damage. I voluntarily assume these risks on behalf of myself and/or my child and agree to release, indemnify, and hold harmless the Studio/Company, Hannah Taylor-Kormondy, and their respective owners, teachers, staff, instructors, volunteers, agents, and representatives from any claims, demands, or liabilities arising from participation in Studio/Company programs or activities, except in cases of gross negligence or willful misconduct.

I understand that the Studio/Company may offer or participate in programs, performances, competitions, events, and activities at locations other than the Studio/Company premises. This waiver applies to participation at all such locations, including but not limited to theaters, competition venues, convention centers, parks, schools, community centers, event venues, and other public or private locations.

I understand that the Studio/Company is not responsible for my child once they leave the supervised area or premises at any Studio/Company activity, whether on or off-site. This includes, but is not limited to, leaving the Studio/Company building, event venue, performance or competition venue, sidewalks, parking lots, surrounding areas, or other designated activity areas. I acknowledge that the Studio/Company is not responsible for any injury, illness, property damage, or other incident that occurs after my child leaves the supervised area, regardless of whether the child leaves intentionally, accidentally, independently, or without the knowledge or permission of a parent/guardian. I accept responsibility for knowing my child’s whereabouts and for ensuring appropriate supervision once my child leaves the supervised area.

I further grant permission for my child to be photographed or recorded during classes, rehearsals, performances, competitions, events, and other Studio/Company activities, whether on or off-site. I authorize the Studio/Company to use such media for marketing, promotional, and educational purposes, including social media, websites, advertising, and print materials, without compensation. I release and hold harmless the Studio/Company, Hannah Taylor-Kormondy, and their respective owners, teachers, staff, instructors, volunteers, agents, and representatives from claims related to such use.

Membership & Billing

I understand that monthly tuition is automatically charged on the 1st of each month using a debit/credit card or ACH payment method on file. All customers are required to be enrolled in autopay. Payment by check is acceptable prior to the 1st of the month, if a check s not recieved prior to the 1st the card on file will be charged.

Tuition remains the same regardless of whether a month has 3, 4, or 5 class weeks. Payments not received by the 10th will incur a $25 late fee. Tuition must be paid in full for students to participate in classes. Students with unpaid balances may be restricted from attending until their account is current.

Payment plans may be arranged for extenuating circumstances with management approval. Approved payment plans must be followed as agreed upon, or the student may be withdrawn from classes.

Ongoing Membership & Withdrawal

I understand that membership continues until canceled. To withdraw from a class or cancel membership, written notice must be emailed to SSD by the 15th of the preceding month. Withdrawals submitted after the 15th will still be billed for the following month.

Attendance, Missed Classes & Closures

I understand that tuition is non-refundable for missed classes, including absences due to illness, vacation, weather, or other closures. Approved make-up classes must be completed during the current season. Planned studio closures are not eligible for make-ups.

Absences should be reported through the Family Portal, email, or phone. Repeated no-shows or unsuccessful contact attempts may result in withdrawal.

Dance Productions

I understand that students enrolled in ongoing weekly youth dance classes are automatically enrolled in SSD's annual Holiday Production and Spring Recital.

Production fees, including a costume, performance experience, and show gift, are billed on October 1 and March 1. Written notice to opt out must be received by September 15 or February 15, respectively, to avoid production fees.

Regular attendance is expected for performers. Excessive absences may require private lessons or affect performance eligibility. Families agree to follow all theater, venue, and backstage policies.

I understand that professional photography and video may be captured during SSD classes, events, and productions for promotional purposes.

**Refund Policy**

I understand that tuition, program membership/registration fees, and payments for services rendered are non-refundable. Exceptions may be granted at management's discretion. **Any approved refund must be issued within 30 days of the original payment date.** In certain circumstances, SSD may offer an account credit in place of a refund.
