This agreement is accepted electronically by the adult participant or the parent or legal guardian accepting it (the Signing
Adult). Participant means the Signing Adult, when participating, and each minor or adult student listed on the Signing Adult's
Studio Pro account who participates in activities offered, organized, or sponsored by Releve Dance Academy (the Academy). In
consideration for being permitted to participate, the Signing Adult agrees as follows for the Signing Adult and, when applicable,
for each minor Participant.
1. Acknowledgment and Assumption of Risk
Dance and related activities involve physical exertion and inherent risks, including strains, sprains, falls, collisions, contact with other
people, and injuries involving floors, barres, props, costumes, equipment, entrances, exits, stairs, sidewalks, parking areas, and travel
between nearby studio locations. Activities may include classes, rehearsals, private lessons, camps, performances, competitions,
workshops, and studio events, whether held at the Academy or another location.
The Signing Adult understands that injuries may occur even when reasonable care is used. The Signing Adult knowingly and voluntarily
assumes all inherent risks of participation and all other risks that may lawfully be assumed, including risks arising from the ordinary
negligence of a Released Party, to the fullest extent permitted by Missouri law.
2. Health, Fitness, and Disclosure
The Signing Adult represents that each Participant is physically able to participate except for limitations disclosed to the Academy. The
Signing Adult will provide accurate information about allergies, medical conditions, medications, injuries, physical restrictions, and other
information reasonably needed for safe participation, and will promptly update that information in the Studio Pro account or notify the
Academy in writing when it changes.
A Participant should stop and inform an instructor if an activity causes pain, illness, dizziness, or other concerning symptoms. The
Academy may restrict participation or request medical clearance when reasonably necessary for safety. The Academy does not provide
medical advice and cannot guarantee that participation is appropriate for a particular person.
3. Release of Liability and Covenant Not to Sue
To the fullest extent permitted by applicable law, the Signing Adult releases and agrees not to sue Releve Dance Academy and its
owners, members, officers, employees, instructors, independent contractors, volunteers, agents, representatives, event personnel, and
facility providers (collectively, the Released Parties) for claims arising from a Participant's presence at or participation in Academy
activities, including claims for personal injury, illness, death, property damage, property loss, or theft caused in whole or in part by a
Released Party's ordinary negligence.
This release does not apply to gross negligence, reckless or intentional misconduct, or any right or claim that cannot lawfully be waived.
4. Parent or Guardian Consent for Minors
If a Participant is under eighteen years of age, the Signing Adult represents that the Signing Adult is the Participant's parent or legal
guardian or is otherwise legally authorized to provide this consent. The Signing Adult authorizes the minor's participation, accepts this
agreement individually and on behalf of the minor to the fullest extent permitted by law, and accepts responsibility for complying with the
Academy's policies and instructions concerning the minor.

5. Emergency Medical Authorization
If a Participant becomes ill or injured and the Signing Adult cannot be reached promptly, the Signing Adult authorizes the Academy and
its personnel to provide or obtain reasonable first aid, contact emergency services, arrange emergency transportation, and authorize
evaluation or treatment reasonably believed necessary under the circumstances. This authorization does not require the Academy to
provide medical care or transportation.
The Signing Adult is responsible for all resulting medical and transportation costs. The Academy may rely on the emergency-contact and
health information maintained in the family's Studio Pro account. The Signing Adult is responsible for keeping that information accurate
and current. An emergency contact is not required to be a physician.
6. Studio Rules and Conduct
The Participant and family agree to follow studio policies and reasonable staff directions. Releve Dance Academy may restrict access,
suspend participation, or end enrollment when conduct threatens safety, becomes abusive or harassing, or repeatedly disrupts
instruction or studio operations.
7. Parking Areas, Sidewalks, and Nearby Streets
The Signing Adult is responsible for supervising minors and safely navigating parking areas, sidewalks, entrances, exits, and streets on
or adjacent to Academy locations, including travel between nearby studio spaces. To the fullest extent permitted by law, the release in
Section 3 applies to claims arising in those areas or during that travel, including claims based on ordinary negligence of a Released
Party.
8. Indemnification for the Participant's Conduct
To the fullest extent permitted by law, the Signing Adult agrees to indemnify and hold the Released Parties harmless from third-party
claims, losses, or reasonable expenses caused by the Signing Adult's or Participant's intentional or reckless misconduct or material
violation of studio safety rules. This section does not require indemnification for a Released Party's own gross negligence, reckless or
intentional misconduct, or any liability that cannot lawfully be shifted.
9. Media Permission and Photo Opt-Out
The Signing Adult grants Releve Dance Academy permission to photograph or record the Participant during classes, rehearsals,
performances, and studio events and to use those images or recordings for studio websites, social media, advertisements, and other
promotional materials without compensation.
Families who do not grant this permission must select Photo Opt-Out for the applicable student in the Studio Pro Parent Portal under
Info > Waivers & Policies. The preference may be changed through the Parent Portal at any time. The Academy will make reasonable
efforts to honor the current preference after it is recorded, but materials created or published before a preference change may remain in
use where removal is not reasonably practicable.
10. Enrollment Packet, Studio Policies, and Financial Terms
The Signing Adult acknowledges receiving or being given access to the Academy's current enrollment packet and studio handbook and
agrees to comply with the policies, rules, tuition obligations, payment terms, withdrawal requirements, costume and recital obligations,
and other conditions stated there. The Signing Adult understands that the Academy may apply reasonable operational updates
communicated in writing, but a material change to this signed waiver will require a new acceptance when required by law or Academy
policy.
11. Electronic Acceptance, Severability, and Governing Law
The Signing Adult agrees that selecting the acceptance box and providing an electronic signature through Studio Pro constitutes the
Signing Adult's signature and acceptance of this agreement. The Signing Adult certifies that the Signing Adult has read this document,
understands that it includes a release of liability and other contractual obligations, has had the opportunity to ask questions, and accepts
it voluntarily.
If any provision is found invalid or unenforceable, it will be enforced to the maximum extent permitted and the remaining provisions will
continue in effect. This agreement is governed by Missouri law. Headings are for convenience and do not limit the meaning of the
provisions.
BY ELECTRONICALLY ACCEPTING THIS AGREEMENT, THE SIGNING ADULT CONFIRMS AUTHORITY TO
ACCEPT IT FOR THE SIGNING ADULT AND EACH APPLICABLE MINOR PARTICIPANT AND
UNDERSTANDS THAT IT AFFECTS LEGAL RIGHTS.
