JDS Waiver for 2025–2026 Dance SeasonPlease check the following upon agreement to the statements below.
____ Registration FeeA $35 registration fee is due for each dancer upon registering for classes. Payment can be made in the form of Venmo, Zelle, cash, or check only. This fee is non-refundable.
____ Payment PoliciesTuition must be paid by the 1st of every month via cash, check, Venmo, or Zelle. Tuition is non-refundable. Payments made after the first week of the month are subject to a $25/week late fee. All returned checks are subject to a $45 fee. Tuition is non-refundable. Payments made after the first week of the month are subject to a $25/week late fee.
____ Release of LiabilityAll students attending class at Jacqueline's Dance Studio do so at their own risk. Owners and instructors are not liable for damages resulting from personal injuries sustained by the students on the premises.
____ Dance AttireAll students must wear proper dance attire during class. I understand that wearing improper attire will prevent me from participating in that class. Details on required dance attire can be found on our website.
____ Recital Costumes and Recital T-shirtAll students are responsible for purchasing costume(s) and a recital t-shirt for our annual recital. Costume prices TBA. Recital t-shirts are $30 per dancer and will be included in the recital fee. All costume payments are due by November 10, 2025. Costume fees are non-refundable.
____ Recital FeeThis fee is per family and the cost is $60. This includes one recital finale shirt, a digital copy of both recital shows, and the venue fee. If you have multiple dancers, there is an additional $30 fee per dancer to cover the additional recital t-shirt.
____ Photo/Video Release (NOT required)I give Jacqueline's Dance Studio permission to take pictures and videos of my child to be used for advertising purposes. An exception applies for competition team members: JDS may display and publicize videos of competition routines for promotional purposes, but no identifying information (such as names or ages) will be included.
____ Code of ConductI agree to uphold the values and policies of Jacqueline’s Dance Studio, and to treat all instructors, staff, dancers, and families with kindness and respect both in and out of the studio.
