I, the parent/guardian of the above student(s), acknowledge that participation in dance is potentially dangerous and the is an inherent risk of injury involved.
In allowing my child to participate in Alamo City Arts Academy activities, I hereby assume all the risks associated with the performing arts. I understand the importance of myself and my child following the instructions and rules set by their instructor/s, and I agree to release Alamo City Arts Academy and its employees of any and all liability which may arise as a result of my child’s participation in activities at Alamo City Arts Academy.

PHOTOGRAPHY
Parent Release Form for Photography and Videography
I, the undersigned, give permission for Alamo City Arts Academy to use video footage and / or photographs of my child/ward, the above acknowledged student(s). This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs, and/or video of my child for use in materials that include, but may not be limited to:
- printed materials (e.g. - brochures and newsletters)
- online and offline advertising and promotion
- videos and digital images such for use on social media.
By signing this form, I acknowledge that I am giving unrestricted permission for my child’s image to be used in print, video, and digital media. I agree that these images may be used by Alamo City Arts Academy for a variety of purposes and that these images may be used without further notification. I do understand that any identifying information including surname and location will not be used in conjunction with any video or digital images.

I, the parent/guardian/self of the enrolled individual, acknowledge that by registering with Alamo City Arts Academy, the student is committed to the full 2024-2025 season, ending on May 31, 2025. Should the student choose to withdraw for any reason, I understand that written notification must be submitted via email to info@alamocityarts.org by the 25th of the month to prevent further charges. Additionally, I acknowledge that Alamo City Arts Academy does not issue refunds for classes.

ASSUMPTION OF RISK, RELEASE OF LIABILITY & HOLD HARMLESS AGREEMENT

I, the parent/legal guardian of the enrolled student, understand and acknowledge that participation in dance, music, theatre, performing arts, rehearsals, performances, classes, workshops, and related activities involves inherent risks of injury. These risks may include, but are not limited to, falls, collisions, muscle strains, sprains, fractures, and other injuries associated with physical activity, movement, equipment, facilities, costumes, props, and performance environments.

I voluntarily allow my child to participate in activities offered by Alamo City Arts and its programs and acknowledge and assume the inherent risks associated with that participation.

I agree that my child and I will follow all applicable safety instructions, policies, procedures, and directions provided by instructors, staff, and representatives of Alamo City Arts.

In consideration for allowing my child to participate, I agree, to the fullest extent permitted by Texas law, to release and hold harmless Alamo City Arts, Alamo City Arts Academy, Alamo City Dance Company, Alamo City Stage Company, Alamo City Mariachi Academy, Alamo City Ballet Folklórico, and their respective directors, officers, instructors, employees, staff, volunteers, contractors, and agents from claims arising from the ordinary negligence of the released parties in connection with my child’s participation in classes, rehearsals, performances, programs, or related activities, including activities occurring on or off Alamo City Arts premises.

This release is not intended to waive any rights or claims that cannot legally be waived under applicable law.

I acknowledge that I have read and understand this Assumption of Risk, Release of Liability & Hold Harmless Agreement and voluntarily agree to its terms on behalf of myself and my minor child.

PHOTOGRAPHY, VIDEO & MEDIA RELEASE

I, the parent/legal guardian of the enrolled student, grant permission to Alamo City Arts and its affiliated programs, including Alamo City Arts Academy, Alamo City Dance Company, Alamo City Stage Company, Alamo City Mariachi Academy, and Alamo City Ballet Folklórico, to photograph, video record, and/or otherwise capture my child’s image, likeness, voice, and performance while participating in classes, rehearsals, performances, events, productions, and other Alamo City Arts activities.

I authorize Alamo City Arts to use these photographs, recordings, and other media for legitimate educational, artistic, archival, promotional, advertising, fundraising, and publicity purposes. This may include, but is not limited to:

• Websites and social media
• Printed brochures, flyers, posters, programs, and newsletters
• Digital and print advertising
• Promotional and fundraising materials
• Press releases and media coverage
• Videos, photographs, and other digital content
• Documentation and archival materials

I understand that these materials may be published publicly and may remain available after my child is no longer enrolled or participating with Alamo City Arts.

I understand that my child will not receive compensation for the use of these photographs, recordings, or other media and that Alamo City Arts is not required to notify me or obtain additional approval before each authorized use.

For the privacy and safety of our students, Alamo City Arts will not publish a minor student’s personal contact information, home address, school information, or other sensitive identifying information. A student’s name may be used when reasonably appropriate, such as in performance programs, cast announcements, awards, recognition, or other arts-related publicity.

By accepting this release, I acknowledge that I have read and understand its terms and voluntarily grant the permissions described above.

Withdrawal & Refund Policy

I, the parent/guardian/self of the enrolled individual, acknowledge that by registering with Alamo City Arts Academy, the student is committing to the full season of instruction.

Should the student choose to withdraw for any reason, written notification must be submitted via email to info@alamocityarts.org no later than the 25th of the month to avoid being charged tuition for the upcoming month.

Non-attendance does not constitute withdrawal from a class. Students will remain enrolled, and tuition will continue to be charged, until the Academy receives written notification of withdrawal in accordance with this policy.

Alamo City Arts Academy does not issue refunds or credits for missed classes or for tuition already charged prior to receiving timely withdrawal notification.
