Agreement to Participate in Groove Kidz:
Participation in physical activity can be inherently dangerous and it is impossible to ensure the safety of all participating individuals. Although many risks can be avoided, I understand that not all risks can be and release Groove Kidz, LLC from responsibility for any injury that my child might incur during the course of normal activity/play. I further understand that in the event of a medical emergency, Groove Kidz will call EMS to render assistance and that I will be financially responsible for any expenses involved.
Your child will remain under the care, direction and supervision of the school while receiving instruction from Groove Kidz, LLC. I hereby release and discharge Groove Kidz and it's members from all actions, claims, or demands. In consideration of being permitted to participate, on behalf of myself, my family, my heirs, and my assigns, I hereby release Groove Kidz, LLC, its agents, and its employees from liability for injury, loss, or death to the above-mentioned participant while using any facility or equipment or in any way associated with participating in the activity of ballet/tap, modern, hip hop, creative movement & gymnastics now or in the future, resulting from the ordinary negligence of Groove Kidz, LLC, its agents and employees.
