I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.

Liability

Ritmo Canela Academy of Performing Arts is not responsible for accident, injury to persons and loss or damage to their property, whether on the premises of the studio or off the premises, or in connection with your classes, training, rehearsals, performances, and travel to and from off premises, rehearsals and performances. It is required that all dance students are covered by their own family insurance policy. I acknowledge that there are risks of injury in participating in these activities and knowingly and freely assume all such risks and hereby release and hold harmless Ritmo CanelaAcademy of Performing Arts LLC, it’s officers, agents, contractors, employees, owners, and /or owners and lessors of premises and equipment.

I understand that as a parent or guardian I will be contacted if medical attention is required during class time. If I cannot be reached, I hereby authorize an director/instructor of Ritmo Canela Academy of Performing Arts LLC, to arrange for treatment as necessary. Each student may decline to participate in any activity that may be deemed as harmful and is responsible for informing the instructor of any physical limitations, which may prevent full participation in class. I will not hold Ritmo Canela Academy of Performing Arts LLC., its affiliates, its faculty, directors, administrators, employees or representatives liable for any costs resulting from any injuries sustained or illnesses contracted or responsible for any article(s) lost or stolen while a student is participating in a Ritmo Canela Academy of Performing Arts LLC. program, except in the case of proven negligence.

I hereby acknowledge that I have read and agree to the terms and conditions of this Waiver of Liability.

For the safety of our dancers, photos and videos are allowed only in designated areas of our studio. Choreography is owned by Ritmo Canela Academy of Performing Arts LLC, and is not to be posted on social media. Negativity on social media (public or private) by dancers or their family members will not be tolerated and may be cause for dismissal. Upon enrollment, all dancers grant permission to Ritmo Canela Academy of Performing Arts to use their image/likeness online, in print, or on television to promote the studio or offer virtual and online instruction. This usage may include (but is not exclusive to) displaying publicly, distributing or publishing photographs and/or video for use in materials that include, but may not be limited to printed materials (i.e. - brochures, newsletters), online and offline ads.

I hereby authorize Ritmo Canela Academy of Performing Arts LLC, to utilize my childs photograph or video image in promotional materials for Ritmo Canela Academy of Performing Arts LLC indefinitely. Promotional materials are defined as brochures, advertisements, the organization’s official website and promotional videos.

I, the undersigned, hereby hold harmless Ritmo Canela Academy of Performing Arts LLC and anyone associated with it for any injuries sustained by the student mentioned below. I acknowledge that I am fully responsible for the student before and immediately after their class. I grant permission for any faculty member of Ritmo Canela Academy of Performing Arts LLC to take necessary steps to obtain emergency medical care for the student listed below. In consideration of your acceptance of this enrollment, I, intending to be legally bound, waive, release, and forever discharge all rights and claims against Ritmo Canela Academy of Performing Arts LLC and any involved personnel for damages or injuries sustained by me or my child during classes, on studio premises, or at any external studio activities, including showcases, workshops, performances, or visits to nearby businesses.

I authorize Ritmo Canela Academy of Performing Arts LLC to use photos and videos taken of my child during dance classes or events for marketing and promotional purposes.

I understand that enrollment at Ritmo Canela Academy of Performing Arts LLC is continuous from month to month unless written notice is provided via email by the 10th of the month. After the 10th, the next month's tuition will still apply.

I agree to pay all tuition and fees at the beginning of each month and understand that there are no refunds for classes missed due to absences, bad weather, or holidays. Additionally, I acknowledge that late fees will be applied for any unpaid tuition or fees not received by the required due date.

I agree to abide by the policies and regulations of Ritmo Canela Academy of Performing Arts Performance Program Handbook, as well as any amendments or modifications made at the sole discretion of the Academy.
