2025-2026 STUDIO POLICY/AGREEMENT FORM!
_____ REGISTRATION FEE of $60 per family is due at registration.
_____ RECITAL FEE of $100 per dancer is non-refundable and due at registration.
_____ ALL FEES are non-refundable
_____ MONTHLY TUITION is due by the 15th of each month. A Late Fee of $15 will be added after the 15th of each month.
_____ ALL PAYMENTS may be made by Credit Card, Debit Card (4% fee charged on cards), Cash, or Check ($30 return check fee). Can also be paid through the Parent Portal. Auto-Pay is also available through the parent portal.
_____ TUITION must be current to participate in classes.
_____ COSTUME PAYMENTS are $100 per individual class. All Kinder Combo class costume fees are $140. Any student withdrawing after November 1st will be responsible for entire costume balance. Late fees will be added if full payment is not paid by November 15. Student withdrawal prior to the recital and after costumes have been ordered will forfeit the costumes.
_____ DANCE ATTIRE Students are required to wear appropriate shoes, leotards, tights, or other dance wear for each class. Hair should be pulled up and away from face (no beads or balls in the hair are allowed) T-shirts, shorts, jeans, street clothes and street shoes are NOT allowed in class. Any students not properly attired may not be allowed to participate in class. IF YOUR DANCER COMES TO CLASS WITHOUT A HAIR TIE, A $1 CHARGE WILL BE ADDED TO YOUR ACCOUNT FOR A “HAIR TIE KIT” IF PARENT NOT PRESENT TO PROVIDE ONE.
THIS KIT INCLUDES A BAG WITH TWO HAIR TIES AND 3 BOBBY PINS.
_____I authorize KDA staff to assist my child with bathroom visits as needed during their time at the studio. I understand that staff will help ensure my child's safety and privacy, but I am responsible for addressing any specific toileting needs, including the management of pull-ups or diapers.
_____ CHEWING GUM, FOOD, AND CELL PHONES are NOT allowed in class. NO FOOD IN THE LOBBY!
_____ ATTENDANCE is important to student progression. All students are expected to participate in class unless there is a written note/ message explaining why they must observe. Please call the studio office, send a message on Facebook, or email janm@kinstondance.com if a student is ill and cannot attend.
_____ WITHDRAWAL POLICY states that a student that wishes to withdraw must submit a written notice to Kinston Dance Academy. Verbal acknowledgment is not acceptable.
_____ SAFETY is our utmost priority. For the safety and security of all our dancers and families, KDA reserves the right to use video recording devices within the premises as needed to help maintain a safe and welcoming environment for everyone.
_____ KDA observes the same holiday schedule as the Lenoir County Schools. The studio is OPEN on teacher workdays. Notice will be sent prior to any other closings. There are no deductions for vacations, bad weather days or short months. There is no extra charge for months with five class weeks.
_____ INCLEMENT WEATHER (such as major storms, snow, ice) may cancel classes. When feasible, we follow the Lenoir County Schools schedule. Please check emails, Facebook, Instagram or call the studio for closing announcements.
_____ PLEASE bring and/or pick up your child promptly before and after his/her class time. Parents will assume responsibility for damage to property caused by unattended children. Kinston Dance Academy is not liable for any injuries your child may receive due to his/her negligence. Children under the age of 14 are not allowed to enter or exit the building alone. Staff members are not responsible for taking students to and from vehicles.
_____ RELEASE As additional consideration for the students instruction, the undersigned hereby releases Kinston Dance Academy from liability for injuries of any kind to the person or property of the student which may occur while participating in activities of the studio. The undersigned further agrees to inform Kinston Dance Academy in the event any claims are asserted against it arising from the students participation in the activities of the studio.
_____ I release media rights for KDA to use any photos or videos of my child for publicity.
