Annette's School of Dance Inc. — Participant Waiver and Release of Liability (2026–2027 Season)
By registering at Annette's School of Dance Inc. ("the Studio"), you agree to the following terms and conditions. This waiver must be read, understood, and agreed upon by the participant and/or the participant's parent or legal guardian prior to registration for the 2026–2027 season. In this waiver, "activities" means all dance and acro classes, camps, workshops, rehearsals, competitions, recitals, performances, and related events run by the Studio.
1. Assumption of Risk
I acknowledge that participating in dance and acro activities involves inherent risks, including but not limited to physical injury, falls, strains, and other unforeseen incidents, and that acro in particular involves inverted and weight-bearing movements. I voluntarily assume all risks associated with participation, understanding that Annette's School of Dance Inc. will take reasonable precautions to ensure a safe environment.
2. Release of Liability
I hereby release, waive, and discharge Annette's School of Dance Inc., its owners, employees, instructors, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action, including those arising from the negligence of the Studio, its owners, employees, or instructors, related to any loss, damage, or injury, including death, that may be sustained by the participant, or to any property belonging to the participant, while participating in the activities.
3. Parent/Guardian Personal Waiver
If I am signing as the parent or legal guardian of a minor participant, I also release and waive, in my own personal capacity, any and all claims I may have against Annette's School of Dance Inc., its owners, employees, instructors, volunteers, and agents arising out of the participant's involvement in the activities.
4. Indemnification
I agree to indemnify and hold harmless Annette's School of Dance Inc., its owners, employees, instructors, volunteers, and agents from any claims, damages, or expenses (including reasonable legal costs) brought by or on behalf of the participant arising out of the participant's involvement in the activities.
5. Supervision and Studio Responsibility
The Studio is responsible for supervising a participant only during that participant's scheduled class time. The Studio is not responsible for a participant before or after their scheduled class, including during breaks or while waiting between classes. Parents and guardians are responsible for their child at all other times while on or around the premises.
6. Medical Treatment
I give permission to Annette's School of Dance Inc., its owners and operators, to seek medical treatment for the participant in the event they are unable to reach a parent or guardian. I confirm that I have disclosed, through the registration process, any physical or mental health concerns, restrictions, allergies, or critical medications (such as an EpiPen or inhaler) relevant to the participant, and otherwise declare the participant to be in good physical and mental health. In case of an emergency, I consent to the participant being transported to the hospital.
7. Photographic Release
I grant permission for Annette's School of Dance Inc. to take and use photographs or videos of the participant during activities for promotional purposes, including but not limited to social media, print, and online publications, without compensation or further approval.
8. Code of Conduct
I agree that the participant will adhere to the code of conduct established by Annette's School of Dance Inc., which includes respecting instructors, fellow participants, and the studio environment. I understand that failure to comply may result in dismissal from the Studio without refund.
9. Financial Responsibility and Studio Policies
I acknowledge that I have read and agree to the Studio's registration, tuition, withdrawal, and refund policies as set out in the current Studio Handbook, and I understand that registration fees and any costume or event deposits may be non-refundable.
10. Governing Law and Severability
This waiver is governed by the laws of the Province of Saskatchewan. If any part of this waiver is found to be unenforceable, the remaining provisions remain in full force and effect.
11. Agreement to Terms
By registering for activities at Annette's School of Dance Inc., I confirm that I am the participant or the participant's parent/legal guardian with authority to sign on their behalf, and that I have read, understood, and agree to the terms and conditions outlined in this waiver.
