Participant Waiver and Release of Liability
By registering at Annette’s School of Dance Inc., you agree to the following terms and conditions. This waiver must be read, understood, and agreed upon by the participant and/or the participant's parent or legal guardian prior to registration.
1. Assumption of Risk
I acknowledge that participating in dance and acro activities involves inherent risks, including but not limited to physical injury, falls, and other unforeseen incidents. I voluntarily assume all risks associated with participation in dance classes, understanding that Annette’s School of Dance Inc. will take reasonable precautions to ensure a safe environment.
2. Release of Liability
I hereby release, waive, and discharge Annette’s School of Dance Inc., its employees, instructors, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant, or to any property belonging to the participant, while participating in the dance camp or related activities.
3. Medical Treatment
The undersigned gives permission to Annette's School of Dance Inc., its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical or mental health concerns, restrictions, or conditions and/or declare the participant to be in good physical and mental health. In case of an emergency I consent to my child being transported to the hospital.
4. Photographic Release
I grant permission for Annette’s School of Dance Inc. to take and use photographs or videos of the participant during camp activities for promotional purposes, including but not limited to social media, print, and online publications, without compensation or further approval.
5. Code of Conduct
I agree that the participant will adhere to the code of conduct established by Annette’s School of Dance Inc., which includes respecting instructors, fellow participants, and the studio environment. I understand that failure to comply may result in dismissal from the studio without refund.
6. Agreement to Terms
By registering for dance classes at Annette’s School of Dance Inc., I confirm that I have read, understood, and agree to the terms and conditions outlined in this waiver.
