Medical Authorization:
I, the undersigned parent or guardian, hereby give my consent for the above-named child to attend and
participate at Nadia’s School of Dance LLC. I certify that my child is in good health and is capable of participating physically in this program. I understand the risk involved in this type of activity and I waive any responsibility on
behalf of the dance school and any individual for personal injuries, which may result during participation or during breaks.

Covid-19 Agreement:
I release Nadia's School of Dance LLC, its instructors, independent contractors, and all other associates from
liability for harm, theft, or injury that may be suffered by me and/or members of my family traveling to or from or during participation
in activities and programs sponsored by Nadia's School of Dance LLC. I here by acknowledge that I am voluntarily assuming full
responsibility for all risks of physical injury arising out of active participation in a dance class or other dance related activities. I
acknowledge the contagious nature of COVID-19 and other contagious diseases and viruses and voluntary assume the risk
that I and/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure
or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming
exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions, or
negligence of myself and others, including, but not limited to, employees, independent contractors, volunteers, and program
participants and their families. I release Nadia's School of Dance LLC, its instructors, independent contractors, and all associates from
liability for harm, injury or death pertaining to COVID-19 and other contagious diseases and viruses. I understand that there are NO
refunds for registration fees. I understand that if my dancers class gets cancelled due to weather, sickness, or any issues arising
from COVID-19 there is no refund for these missed classes.

Liability Waiver:
I understand that all precautions will be taken to prevent any accidents, I hereby release Nadia’s School of Dance LLC,
it’s agents, or employees from any accidents involving my child. In case of an emergency, I hereby authorize a
representative of Nadia’s School of Dance LLC to seek medical attention for my child.

Nadia's School of Dance LLC (NSD) has my permission to use any video/photographs taken during classes, rehearsals, performances, competitions, and other dance related activities for multimedia announcements, advertising and display. I understand that these videos/photographs are property of NSD and may be utilized at the discretion of NSD. I recognize that my child's name and personal information will not be associated with these images.

I recognize the risk of injury in dance programs. This student is participating upon the agreement and understanding that I am hereby waiving and releasing Nadia's School of Dance LLC, it's owners, employees, contractees and choreographers from and against any and all claims, cost, liabilities, expenses or judgements, including attorney fees and court costs arising from participation in this dance program.
