Liability: I, the undersigned, for myself and on behalf of my child and in consideration of my or my child’s participation at the Barbara Hammond’s School of Dance, Inc. (the “Schoolâ€), and with the full and complete understanding associated with the risks related to dancing, gymnastics, Pilates, and any other activities and classes offered by the School, do hereby release, acquit and forever discharge, to the full extent allowed under applicable law, the School and its agents, directors, officers, owners, employees, affiliates, and their respective successors and assigns (collectively, the “Released Partiesâ€), from and against any and all claims, damages, liabilities, actions, and expenses (including without limitation, attorney’s fees) of any nature caused by or related to personal injury and/or property damage, known or unknown, which may result from or arise out of my or my child’s participation or preparation of the aforementioned activities, or for any other activities associated with the School, whether occurring at the School’s studio or at any other venue attended by me or my child, and regardless of whether such injury and/or damage arises before, during or after the aforementioned activities. Further, I agree to indemnify, defend and hold harmless the Released Parties for any and all claims, damages, liabilities, actions and expenses (including without limitation, reasonable attorneys’ fees) arising from the personal injury and/or property damage, however sustained or arising (including without limitation, by the negligence of the Released Parties), resulting from my or my child’s participation in the aforementioned activities.
I have read this release and authorization and fully understand its terms. I execute it voluntarily and without any inducement or assurance of any kind on the day and year stated below.
