EMPOWER DANCE INDUSTRY WAIVER
PLEASE NOTE ALL CLASSES ARE CHARGED MONTHLY. SOME MONTHS HAVE A DIFFERENT NUMBER OF WEEKS BUT TUITION WILL BE THE SAME EACH MONTH.
TUITION IS DUE BY THE 7TH OF EACH MONTH.
IF A STUDENT MISSES CLASSES (not by fault of the studio), FULL MONTHLY TUITION IS STILL DUE (unless it is a drop-in rated class).
In order to drop a class you must notify the studio 30 days prior to dropping to avoid being charged for the following months tuition.
Empower Dance Industry will not issue any refunds, but can credit accounts for a future purchase, such as camps, tuition, or merchandise.
Students dropping classes after February 2025 will be charged tuition for full season.
By agreeing below you are agreeing to all of the rules, & regulations of Empower Dance Industry & the director. I agree to allow my child to participate in the dance classes selected and understand that Empower Dance Industry is not responsible for the loss of any property, accidents, or injuries sustained at Empower Dance Industry. I agree EDI has the right to use any photos and /or videos for marketing purposes. I also agree and comply with the Covid-19 safety form, and know that should Empower Dance industry close at any time due to Covid-19 all classes should continue online using Zoom. Assumption of the Risk and Waiver of Liability Relating to Injury and/or Communicable Diseases Including Coronavirus/COVID-19 The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person- to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Empower Dance Industry, LLC has put in place preventative measures to reduce the spread of COVID-19; however, EDI cannot guarantee that you or your child will not become infected with COVID-19. Further attending EDIâ€TMs classes or events could increase your risk of contracting COVID-19. By this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I (or my child) may be exposed to or infected by COVID-19 by attending classes or events at EDI and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed or infected by COVID-19 may result from the action, omissions, or negligence of myself and others, including, but not limited to, EDI employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility of any injury to myself (or my child), (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at EDI or participation in EDI programing. On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless EDI, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of EDI, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any EDI program.
