EMPOWER DANCE INDUSTRY POLICIES & WAIVER
Tuition & Payment Policies
* Tuition is charged monthly and is a flat monthly rate. Some months may have a different number of class weeks; however, tuition remains the same each month.
* Tuition is due by the 7th of each month.
* Payments may be made online by credit card through the parent portal, by Venmo (@courtney-moniz-2), or in person by cash or check.
* Late fees may be added to accounts not paid by the due date.
* Returned checks are subject to an additional fee.
* If a student misses class for any reason not caused by the studio, full tuition is still due (unless enrolled in a designated drop-in class).
* Tuition and fees are non-refundable. Account credits may be issued at the discretion of Empower Dance Industry for future tuition, camps, classes, or merchandise purchases.
* To withdraw from a class, written notice must be provided at least 30 days prior to withdrawal to avoid being charged the following month’s tuition.
* Students withdrawing after February 1st of the current dance season may be responsible for the remaining season tuition and/or fees.
Attendance & Class Policies
* Students are expected to arrive on time and prepared for class.
* Proper dance attire and shoes are required for participation and safety.
* Hair must be securely pulled back for all classes.
* Empower Dance Industry reserves the right to dismiss any student from class due to disruptive, unsafe, disrespectful, or inappropriate behavior.
* Parents and students are expected to treat staff, instructors, students, and studio property with respect at all times.
Photo & Video Release
I understand and agree that Empower Dance Industry may use photographs and/or videos of students taken during classes, rehearsals, performances, competitions, camps, or studio events for promotional, advertising, social media, website, and marketing purposes without compensation.
Assumption of Risk & Liability Waiver
I understand that participation in dance, acrobatics, tumbling, stretching, conditioning, and related physical activities involves inherent risks of injury, including but not limited to sprains, fractures, muscle injuries, concussions, permanent disability, or death.
By enrolling in classes and/or participating in activities at Empower Dance Industry (“EDI”), I voluntarily assume all risks associated with participation for myself and/or my child.
I hereby release, waive, discharge, and hold harmless Empower Dance Industry, LLC, its owners, directors, instructors, staff members, volunteers, contractors, and affiliates from any and all claims, liabilities, demands, damages, costs, expenses, actions, or causes of action arising out of or related to any loss, injury, illness, damage, or accident that may occur while participating in EDI classes, rehearsals, performances, competitions, camps, events, or while on studio premises.
I understand that Empower Dance Industry is not responsible for lost, stolen, or damaged personal belongings.
Medical Emergency Authorization
In the event of an emergency, I authorize Empower Dance Industry staff to seek emergency medical treatment for myself or my child if I cannot be reached immediately. I understand that I am financially responsible for any medical treatment provided.
Illness & Communicable Disease Acknowledgement
I understand that participation in classes and activities at Empower Dance Industry may involve exposure to communicable illnesses. By participating, I voluntarily assume these risks and agree not to hold Empower Dance Industry, LLC, its staff, or affiliates liable for illness or related claims arising from participation in studio activities.
Agreement
By checking the registration box during enrollment, I acknowledge that I have read, understood, and agree to comply with all policies, rules, regulations, waivers, and terms of Empower Dance Industry. I understand that checking the box serves as my electronic signature.
