Electronic Signature Agreement

By selecting the “I Accept” checkbox, I am signing this agreement electronically. I agree that my electronic signature is the legal equivalent of my handwritten signature on this agreement.

I consent to the use of electronic records and signatures in connection with this agreement and acknowledge that I may withdraw this consent at any time by providing written notice.

I understand that electronic records of my acceptance may be retained and are admissible as evidence if a dispute arises.

I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.

Public Grievance Policy

I acknowledge the policies and guidelines regarding the airing of grievances at Danse Élan and agree to the following:

I will make every effort to address concerns through the appropriate internal channels before sharing them publicly. These channels include:
a. Email: sending a detailed description of my concern to INFO@DANSEELAN.COM

b. Meeting: scheduling a private meeting with the studio director or appropriate staff member by contacting INFO@DANSEELAN.COM

I understand that while I may share opinions publicly, I agree not to make false, defamatory, or harassing statements about the studio, its staff, or its students.

I acknowledge and agree to the non-retaliation policy and will not engage in retaliation against any individual who raises a concern in good faith.

I understand and agree that all grievances will be handled with the utmost confidentiality, and the privacy of all parties involved will be respected throughout the resolution process.

By signing below, I acknowledge my understanding and agreement to adhere to these guidelines and to help foster a positive environment at Danse Élan.

Danse Élan LLC – Student Waiver and Release of Liability

Acknowledgment of Policies:

I have received or will obtain a copy of Danse Élan LLC’s policies and agree to carefully read and follow them. I understand that Danse Élan does not give credit or refunds for missed classes, workshops, or camps due to holiday, vacation, illness, weather, or other reasons. I also understand that Danse Élan does not give credit or refunds for costume payments, recital fees, or other applicable fees.

Except as otherwise required by North Carolina law, all tuition, registration, costume, and recital fees are non-refundable.

Release of Liability / Assumption of Risk:

I understand that participation in dance and related activities carries inherent risks of injury or property damage. I voluntarily assume all such risks for myself or my child(ren).

I hereby release, indemnify, and hold harmless Danse Élan LLC, its owners, staff, employees, and representatives from any and all claims, including those arising from the ordinary negligence of Danse Élan, related to participation in any of its programs or use of its facilities.

This release does not apply to claims arising from gross negligence or intentional misconduct.

Emergency Medical Authorization:

I authorize Danse Élan staff and faculty to seek emergency medical treatment for myself or my child(ren) if necessary. I understand that every reasonable effort will be made to contact me in the event of a medical emergency. I agree to hold harmless Danse Élan personnel for any actions taken in good faith while seeking care.

Withdrawal and Refund Policy:

If a student must withdraw, written notice must be provided at least five (5) days prior to the next tuition due date to discontinue automatic payments.

Except as otherwise required by North Carolina law, registration fees, costume fees, and tuition are non-refundable.

Photo and Video Consent"

By signing below, I grant permission for Danse Élan to use photographs or video of myself or my child(ren) for promotional and educational purposes related to the studio’s programs.

Communicable Diseases (Including COVID-19):

I acknowledge that participation in group activities may expose me or my child(ren) to contagious illnesses such as MRSA, influenza, or COVID-19. I voluntarily assume all such risks and accept sole responsibility for any resulting illness, injury, disability, or death.

On my behalf, and on behalf of my child(ren), I release, covenant not to sue, and hold harmless Danse Élan LLC, its employees, agents, and representatives from any claims arising out of such exposure or infection, including those resulting from the ordinary negligence of the studio or its staff.

This release does not apply to claims arising from gross negligence or intentional misconduct.

By signing, I confirm that I have read, understood, and agree to the terms of this Student Waiver and Release of Liability.
