You must be over 18 to register for an account.

I understand and agree to pay the fees listed below.

Registration Fee: $25.00 due at time of registration plus tuition
Monthly Tuition: Due at the time of registration (Please refer to the prices brochure link on the home page for multiple class pricing.)
Costume Deposit: $60 per class participating in the recital (Due Nov 1st, remaining balance will be billed in Spring)
Recital Fee: $100 Due April 1st. (Includes unlimited entries to our recital, one complimentary copy of program, a recital t-shirt, and digital download of recital. +$20 for each additional sibling)

*Late Fee - Late payments will be charged an additional fee of $10 for every 7 days an invoice is overdue.

We require a 30 day notice to drop classes. All fees are non-refundable.

Accounts must be paid in full for dancer to participate in recital. Dancers with balances on their account will not be able to participate in recital.

Tuition invoices will be emailed from Intuit Quickbooks and is due on the 1st of each month.

I give Poquoson Dance Academy permission to photograph my child for the purposes of promoting/publicizing the studio.

I understand that Poquoson Dance Academy will not be held responsible for any bodily injuries sustained while on the premises or for loss or damage to any personal items brought on the premises by dancers or their families. Parents are responsible for notifying Poquoson Dance Academy of any condition that may affect or limit the dancer during class.

In the event of an emergency, I hereby authorize Poquoson Dance Academy to obtain necessary emergency medical treatment for my child, with the understanding that the family will be notified as soon as possible
