POINT OF GRACE DANCE LIABILITY WAIVER FORM 24’

I recognize that the program may involve strenuous physical activity including, but not limited to, muscle strength and endurance training, cardiovascular conditioning and training, and other various fitness activities. I hereby affirm that the child I am signing for is in good physical condition and does not suffer from any known disability or condition which would prevent or limit participation in this program. I affirm that I am a parent or guardian of the child and have the authority to decide to allow this child to practice and participate in the program. I release and agree to save, indemnify and hold Point of Grace Dance and its employees and volunteers harmless from any and all claims, losses,a or expenses arising out of the negligent performance of the program related to or arising from strenuous physical activity including, but not limited to, muscle strength and endurance training, cardiovascular conditioning and training, and other various fitness activities. Indemnification includes all litigation expenses and reasonable attorney’s fees at trial and on appeal. I HEREBY AFFIRM THAT I HAVE READ AND FULLY UNDERSTAND THE ABOVE STATEMENTS.

Photo consent and release: I hereby grant Point of Grace Dance permission to take photographs and videos of my child participating in activities of the studio and use them on social media platforms for publicity or other purposes to advertise the business. I HEREBY AFFIRM THAT I HAVE READ AND FULLY UNDERSTAND THE ABOVE STATEMENTS.
