I understand that activities, especially dance, acro and fitness, involve potential risk of physical injury and related damages. I acknowledge and accept all risk associated with this activity and agree to release Align Dance Training and the directors of this (these) service(s) from all suits, claims, or demands of every kind and character arising out of and in connection with, the service provided by Align Dance Training. I do hereby release Align Dance Training, the owners, instructors, staff and volunteers in charge for all actions, demands or claims that may result because of injury. I further certify that this participant has no ailment or organic defect that would make participation in these activities dangerous to the health of the participant.

All SEASON 6 classes participate in the spring recital. Our 2026 recital is tentatively planned for Mid to Late May 2027. Rehearsals for recital take place at the theater, the week leading up to the performance, and are mandatory to attend in order to perform. A $65 costume deposit is due in November for each class, and costume balances are due in April.

I further certify that this participant has no ailment or organic defect that would make participation in these activities dangerous to the health of the participant.

Align Dance Training may photograph and video students during classes. Photos and videos may be used for advertising purposes and posted on social media, and other publications.
