By initialing here you give your permission for photographs/videos of the enrolled participant to be used for advertising/social media for CSDM. You will not be compensated for use of the photos/videos.

Each class requires a minimum of 4 registered students to remain on the schedule as a group class.
If Enrollment Falls Below 4: Families will be offered the option to continue as a trio, duet, or solo class (at adjusted pricing), or to transfer to another class that fits the dancer's age and level. Schedule is subject to change based on enrollment. CSDM will communicate schedule changes as early as possible.

Tuition is a monthly fee and does not change due to absences or cancellations.
Tuition payments must be received on or before the 1st of each month.
Any payment received after the 1st of the month will be subject to a $20 late fee.
Tuition rates are subject to change with advance notice provided to families.

There are risks of physical injury associated with, arising out of and inherent to the activity of dance. By affixing your initials you acknowledge this risk of injury, and knowingly and voluntarily waive all right and/or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to CSDM, LLC its owner, officers, agents, employees, instructors, subsidiaries, and all affiliated entities

WITHDRAWAL POLICY: We require a 30-day written notice via email prior to the automatic payment on the 1st of each month. Please email cynthia@cynthiasschoolofdance.com to withdraw from classes. After the 30-day notice, we will terminate your monthly auto pay. Please note that if you give less than 30 days notice before the next payment is scheduled, you should expect to make one last tuition payment.
