Assumption of Risk & Release of Liability

I understand that participation in dance, acrobatics, tumbling, stretching, conditioning, performances, camps, competitions, rehearsals, and related activities involves inherent risks, including but not limited to:

Slips and falls
Muscle strains and sprains
Bruises
Broken bones
Head injuries
Permanent disability
Death

I voluntarily allow myself/my child to participate in activities at Ten Feet Tall Dance Studio.

I knowingly assume all risks associated with participation.

To the fullest extent permitted by Texas law, I release and hold harmless Ten Feet Tall Dance Studio, its owners, instructors, assistants, employees, volunteers, landlords, and affiliates from any liability for injuries, illnesses, damages, or losses arising from participation except where prohibited by law.

Medical Authorization

I certify that I/my child is physically able to participate.

I agree to notify the studio of any medical conditions, allergies, injuries, or limitations.

In the event of an emergency, I authorize Ten Feet Tall Dance Studio to obtain emergency medical treatment if I cannot be reached.

I understand that I am financially responsible for any medical expenses incurred.

Illness Policy

Please do not attend class if the student has:

Fever within the past 24 hours
Vomiting or diarrhea within the past 24 hours
Contagious illness
Unexplained rash

The studio reserves the right to send home any student displaying symptoms of illness.

Photography & Video Release

I grant permission for Ten Feet Tall Dance Studio to photograph and record my child during classes, rehearsals, camps, performances, and events.

These images or videos may be used for:

Social media
Website
Advertising
Studio displays
Printed materials

No compensation will be provided.

Drop-Off & Pick-Up

Parents are responsible for ensuring students arrive safely and are picked up promptly.

Ten Feet Tall Dance Studio is not responsible for students before class begins or after class ends unless participating in a supervised activity.

Personal Property

The studio is not responsible for lost, stolen, or damaged personal belongings.

Please label all shoes, clothing, and water bottles.

Tuition & Payment Policy

I understand that:

Tuition is due on the 1st of each month.
A $15 late fee is added after the 7th.
Tuition is NOT prorated for missed classes.
Tuition reserves a student's place in class.
Tuition is non-refundable.
Weather cancellatin are NOT refundable

Make-Up Classes

One make-up class may be offered when space is available for an absence reported in advance.

Missed make-up classes cannot be rescheduled or credited.

Attendance

Regular attendance helps dancers progress.

Students arriving more than 15 minutes late may be asked to observe class for safety reasons.

Behavior Expectations

Students are expected to:

Show kindness and respect.
Listen to instructors.
Follow safety instructions.
Treat the studio and equipment with care.

Repeated disruptive behavior may result in removal from class.

Acro & Tumbling Safety

Acrobatic skills should only be practiced under an instructor's supervision.

The studio is not responsible for injuries resulting from practice outside the studio.

Competition Team (if applicable)

Competition team members agree to follow additional team policies regarding:

Attendance
Rehearsals
Dress code
Competitions
Financial obligations
Sportsmanship
Parent Agreement
