**Parent / Guardian Agreement & Waiver**

By enrolling my child at Precision Institute of Dance, I acknowledge and agree to the following:

I confirm that I have read, understood, and agree to comply with all policies, procedures, expectations, and terms outlined in the Precision Institute of Dance Parent Handbook. I understand that these policies apply to all students and families and are in place to ensure a safe, respectful, and professional training environment.

I acknowledge that dance and physical training involve inherent risks, including but not limited to physical injury. I accept full responsibility for my child’s participation and confirm that my child is physically fit to take part in dance classes and related activities. I agree that Precision Institute of Dance, its directors, staff, contractors, and volunteers are not liable for any injury, illness, or loss that may occur as a result of participation, except where required by law.

I understand that it is my responsibility to disclose any relevant medical conditions, injuries, or special requirements and to keep this information up to date via the Parent Portal.

I agree to abide by all studio policies relating to fees, enrolments, payments, cancellations, credits, behaviour expectations, communication procedures, and student or parent conduct. I understand that failure to comply with studio policies may result in limited participation or termination of enrolment, in accordance with the Parent Handbook.

I acknowledge that all studio communication is delivered via BAND and the Parent Portal, and it is my responsibility to remain informed by ensuring notifications are enabled and details are kept current.

I understand that the Parent Handbook may be updated from time to time and that continued enrolment constitutes acceptance of any updates.

By proceeding with enrolment, I confirm that I have read, understood, and agree to all of the above.

By completing the registration process and creating a profile with Precision Institute of Dance, I acknowledge that I have read, understood, and agree to comply with the current Precision Institute of Dance Parent Handbook, including all policies relating to enrolment, fees, attendance, behaviour, withdrawals, communication, media use, health and safety, private lessons, competitions, and participation in studio activities, and I understand that enrolment, continued attendance, and participation in any classes, events, programs, or services constitutes full acceptance of these policies and any future updates communicated by the studio; I agree to pay all tuition fees, administration fees, and any additional costs associated with my child’s participation, including but not limited to costumes, choreography, private lessons, examinations, workshops, competitions, performances, and events, in accordance with the studio’s fee structure, and I acknowledge that all fees are non-refundable except where required by Australian Consumer Law and remain payable in full regardless of absence, withdrawal, injury, or change in circumstances; I understand that a minimum of two weeks written notice is required for withdrawal and that failure to provide proper notice does not remove or reduce any financial obligation; I acknowledge that enrolment secures a place in a class, not attendance or results, and that placement decisions are at the discretion of the studio; I confirm that my child is physically capable of participating or that I have disclosed any relevant medical conditions and will keep this information updated, and I accept that dance and physical training involve inherent risks, including the risk of injury; to the fullest extent permitted by law, I release and hold harmless Precision Institute of Dance, its directors, staff, and representatives from liability for any injury, loss, or damage sustained during participation in any studio activity except where liability cannot be excluded under Australian Consumer Law; I agree to ensure my child adheres to all uniform, grooming, behaviour, and safety requirements and understand that failure to do so may result in disciplinary action, reassessment, suspension, or termination of enrolment without refund; I acknowledge that if my child participates in a competition team, this constitutes a full-season commitment and all associated fees remain payable in full once committed, and that costumes transfer to family ownership only upon completion of the season and must be returned if withdrawal occurs earlier; I acknowledge that all choreography created by Precision Institute of Dance staff remains the intellectual property of the studio and may not be performed, taught, or used outside of the studio without prior written permission; I consent to the use of photographs and video footage of my child for training, assessment, and promotional purposes unless I provide written notice otherwise, understanding that full exclusion cannot be guaranteed in group settings; I agree to communicate respectfully with the studio through approved channels only, including the BAND app, email, and mobile communication, and acknowledge that social media platforms such as Instagram and Facebook are not approved communication channels; I understand that the parent portal is the primary system for managing enrolments, fees, and communication and that failure to access or review portal information does not exempt me from any obligations; I acknowledge that the studio reserves the right to make changes to timetables, staffing, classes, and programs as required and that such changes do not entitle me to refunds or fee adjustments; I understand that the studio may terminate enrolment or refuse service at its discretion where policies are not followed or where participation is deemed unsuitable, and that no refunds, credits, or compensation will be provided in such circumstances; and by proceeding with enrolment, I confirm that this agreement is binding and forms part of the conditions of enrolment with Precision Institute of Dance.
