PARTICIPANT RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT
I, the undersigned, hereby acknowledge that I am aware of and fully appreciated the nature of the activities of the programs at Riverside Gymnastics Dance & Fitness, LLC. I acknowledge that I understand the nature of the activities that I will be participating in and that I am in proper physical condition and capable of participating in the related activities provided by Riverside Gymnastics, Dance & Fitness, LLC.
In consideration of enrollment at Riverside Gymnastics, Dance & Fitness, LLC’s program, I understand and agree to release, hold harmless, and discharge Riverside Gymnastics, Dance & Fitness, LLC from all claims, costs, liabilities, expenses or judgements, including attorneys’ fees and court costs for any occurrences in connection with any instruction or activity provided by Riverside Gymnastics, Dance & Fitness, LLC. I assume all risks to myself/my child in connection with any instruction and further release Riverside Gymnastics, Dance & Fitness, LLC, its owners and employees from liability for any injury sustained by myself or my child while enrolled in any class/program including all risks reasonably connected with such activity whether foreseen or unforeseen.
I understand that Riverside Gymnastics, Dance & Fitness, LLC is not responsible for my child or other children under my supervision who are left unsupervised in the common areas and areas surrounding the property of Riverside Gymnastics, Dance & Fitness and that Riverside Dance Company will only be supervising my child when he or she is participating in scheduled instruction. I also understand that Riverside Gymnastics, Dance & Fitness is not responsible for personal property that is lost, damaged or stolen while I or my child are at or on Riverside Gymnastics, Dance & Fitness, LLC’s property.
I HAVE FULLY INFORMED MYSELF ON THE CONTENTS OF THIS RELEASE AND HAVE READ PRIOR
TO SIGNING AT MY OWN WILL
Medical Parent / Guardian Release
I/we, the undersigned participant, parent(s) or legal guardian(s) of the minor named above, hereby consent to and approve of his/her participation at Riverside Gymnastics, Dance & Fitness, LLC. I/we read the above release and contract and agree to all of its entirety.
I/we, the undersigned, parent(s) or legal guardian(s) of the minor named above, hereby further give my/our consent to Riverside Gymnastics, Dance & Fitness, LLC, its manager or authorized employee to obtain emergency medical treatment for myself, or my/our child, including transportation to a hospital, clinic, or other emergency medical facility, obtaining medical services. I/we hereby release, indemnify, have and hold harmless Riverside Gymnastics, Dance & Fitness, LLC from all negligence, liabilities, damages or injuries whatsoever, expressly including all principals, officials, or employees, for all such actions in obtaining medical treatment as hereinabove described.
