The High Pointe Performing Arts Studio, Liability Waiver and Acknowledgment of Risk:
READ AND SIGN BELOW REGISTRATION IS INCOMPLETE WITHOUT SIGNATURE AND MUST BE COMPLETED BEFORE CLASS
I understand and agree that in participating in any dance class, workshop, rehearsal, or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of the High Pointe Performing Arts Studio classes, rehearsals, performances, or activities. I also exempt, release, and indemnify McKenzi Walker, dba High Pointe Performing Arts Studio, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by the High Pointe Performing Arts Studio. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold the High Pointe Performing Arts Studio, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, negligence, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights. Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at High Pointe Performing Arts Studio, or participation in programming ("Claims"). On my behalf, and behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless McKenzi Walker, dba High Pointe Performing Arts Studio, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of High Pointe Performing Arts Studio, LLC, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any High Pointe Performing Arts Studio program. Permission is granted to the High Pointe Performing Arts Studio to use photographs of students for publicity purposes.
