I understand that participation in dance classes, rehearsals, performances, competitions, conventions, workshops, camps, private lessons, conditioning classes, and other activities offered by DanceSpace involves inherent physical risks, including but not limited to falls, collisions, strains, sprains, fractures, and other injuries.
I voluntarily choose to allow my child to participate in these activities and acknowledge that such risks cannot be completely eliminated, even with appropriate instruction, supervision, and safety precautions.
Activities Covered
This Assumption of Risk & Liability Waiver applies to all DanceSpace programs and activities, including but not limited to:
Weekly dance classes
Performance Company rehearsals
Private lessons
Solos, duets, and trios
Competitions and conventions
Performances and recitals
Summer camps and workshops
Master classes and guest artist classes
Boot Camps and conditioning classes
Studio-sponsored events and activities
Activities conducted both on and off DanceSpace premises
This agreement also applies to travel between activity locations when coordinated or supervised by DanceSpace staff as part of an official DanceSpace event.
I understand that the policies and expectations outlined in this waiver remain in effect regardless of whether the activity takes place at DanceSpace or at an off-site venue.
Release of Liability
In consideration of my child's participation in DanceSpace programs, I, on behalf of myself, my child, my heirs, executors, administrators, and assigns, hereby release, waive, and discharge DanceSpace, its owners, directors, instructors, employees, assistants, volunteers, independent contractors, choreographers, and facility owners from any and all claims, demands, causes of action, damages, losses, or liabilities arising out of or relating to my child's participation in DanceSpace activities, except to the extent caused by gross negligence or willful misconduct, as permitted by applicable law.
This agreement shall be governed by and interpreted in accordance with the laws of the Commonwealth of Virginia. If any provision of this agreement is determined to be unenforceable, the remaining provisions shall remain in full force and effect.

Emergency Medical Care
I authorize DanceSpace to obtain emergency medical treatment for my child if I cannot be reached.
I authorize DanceSpace to contact Emergency Medical Services (EMS) and arrange ambulance transportation if deemed necessary by DanceSpace staff or emergency personnel.
I understand that I am solely responsible for all medical expenses, ambulance charges, hospital fees, physician services, and any related costs resulting from my child's treatment.
Medical Fitness
I certify that my child is physically able to participate in dance activities.
I understand that it is my responsibility to notify DanceSpace of any medical conditions, injuries, illnesses, or physical limitations that may affect my child's participation.
If my child becomes injured during the season, I understand that DanceSpace may require written medical clearance from a licensed healthcare provider before returning to full participation.
I acknowledge that dance instruction may include appropriate physical contact by instructors, including spotting, positioning, alignment corrections, and demonstrations, to promote proper technique, safe execution of movement, and dancer safety.
This agreement shall be governed by and interpreted in accordance with the laws of the Commonwealth of Virginia. If any provision of this agreement is determined to be unenforceable, the remaining provisions shall remain in full force and effect.

By signing this waiver, you are acknowledging that you have read the policies stated on our website under "Policies" and agree to all terms including: Dress Code, Class Etiquette, Food/Drink, Substitutes and Tuition.
I have carefully read and understand this Assumption of Risk & Liability Waiver.
I acknowledge that participation in dance involves inherent risks and that I voluntarily allow my child to participate in DanceSpace activities.
I understand that this agreement remains in effect for the duration of my child's participation in DanceSpace during the 2026–2027 season.
This agreement shall be governed by and interpreted in accordance with the laws of the Commonwealth of Virginia. If any provision of this agreement is determined to be unenforceable, the remaining provisions shall remain in full force and effect.
