Liability Waiver
I hereby waive and release Thrive School of Dance, its officers, agents, and employees, from and against any claims, suits, losses, or related causes of action for damages, including attorney’s fees and court costs, that may result from injury or death, accidental or otherwise, during, or
arising from my child’s participation in a dance class, workshop, event or performance, including transportation to and from activities associated with the program, and any resulting illness and/or injury. I agree to indemnify and hold harmless Thrive School of Dance from and against any and all such claims. I recognize that the recreational events or activities entered into by my child can be dangerous to the child and I accept those dangers. I understand that if my child is injured, this waiver will be used against me and anyone claiming damage in any legal action because of my child’s injury. I hereby acknowledge that I understand and am familiar with the nature of the activities in which my child will participate at this studio, and affirm that my child is in good health and that my child does not have a physical or emotional condition, past or present, of which I am aware, which would in any way affect my child’s ability to participate. I also understand that I should have health and accident insurance to cover injuries arising from participation. In case of emergency, I give my permission for emergency medical treatment of my child and for transportation to such treatment.
Dress Code Policy
Consistent with our professional approach to dance education, we require our students to adhere to the following dress code:
Base Dress Code (students aged 3 to 9)
We require our younger dancers to wear a solid color leotard and tights. Ballet skirts are allowed for ladies and black shorts or pants are allowed for gentlemen. We prefer pink or flesh tone ballet shoes and black tap shoes. For dancers taking jazz, we prefer black jazz shoes. We ask that all hair be pulled back away from the face. To maintain professionalism and safety, we do not allow bare tummies or legs, or baggy clothing.
Mid-level Dress Code (Intermediate + Advanced Intermediate students):
We require dancers in the intermediate and advanced intermediate levels to wear a solid color leotard and tights. Ballet skirts, dance shorts, solid color leggings, or dance pants are allowed as layers in addition to the leotard and tights. We prefer pink or flesh tone ballet shoes, black tap shoes, and black jazz shoes. Hair must be secured in a bun for ballet and a bun or ponytail for tap and jazz. To maintain professionalism and safety, we do not allow bare tummies or legs, or baggy clothing.
Company Level Dress Code (Junior Company, Senior Pre, Apprentice, and Senior Company dancers):
We require all company dancers to wear a solid color leotard as a base layer for all classes. To maintain professionalism and safety, we do not allow bare tummies or legs, or baggy clothing. Hair must be secured in a bun for ballet and away from the face (bun, ponytail, braids, etc.) for all other styles. See below for style-specific requirements in addition to the mandatory leotard:
Ballet and pointe: Tights are required and skirts are allowed. We prefer pink or flesh tone split-sole ballet shoes.
Jazz: Solid color tights or leggings/pants and form-fitting tops are allowed. We prefer black split-sole jazz shoes.
Tap: Solid color tights or leggings/pants and form-fitting tops are allowed. We prefer black tap shoes.
Modern: Solid color tights or leggings/pants and form-fitting tops are allowed. No shoes are required.
Hip Hop: Solid color tights or leggings and form-fitting tops are allowed. Other layers are allowed at the teacher’s discretion.
Yoga: We allow students to break the dress code for yoga and wear any comfortable clothing that they can freely move in, that does not show their tummies. We recommend leggings and a form-fitting top.
