Enrollment Authorization Agreement Form
Registration Fee: The registration fee is $25 for a single student.
The registration fee is non-refundable.
Monthly Tuition: Is a set amount that includes Costume fees and Recital fees with the set amount.
Tuition: Tuition will be billed automatically on the first day of each month from August through May. Monthly payments are based on an average of four (4) classes per month during the dance year. Some months will be five (5) weeks and some will have three (3) weeks.
Payments: Tuition is due on the 1st of each month. Customers have the only option to pay debit/credit card on the Parent Portal with Auto-pay being mandatory. There is an online service charge of $10.00 when payments bounce or are not processed due to whatever reason. You are responsible for payment for all classes that you are enrolled in, even if absent, until Alyssa’s School of Performing Arts receives a two (2) weeks written notice of withdrawal prior to the end of the month. Missed lessons cannot be used in place of payment; no adjustment or credit will be given for missed classes. Written notice to the date received, payments will be paused. Example: If tuition is processed on the 1st of the month and we received notice on the 5th of the same month, you are responsible for that month’s tuition.
Any changes from monthly tuition enrollment either in the amount of classes, classes enrolled, shall also be made in writing at least (2) weeks prior to the beginning of the month when Tuition is posted. Example: Tuition is posted on the 1st and you send written notice of dropping a class on the 5th of the month, you are responsible for that month’s posted tuition.
A make-up class can be requested for missed classes. You have 30 days from the missed date to complete a make-up class. We do not prorate tuition or complete refunds due to missed classes. No refunds for classes missed including scheduled holidays.
Code of Conduct: Appropriate and respectful behavior must be exhibited in our school particularly during class and rehearsals. This includes inappropriate behavior during class and no cell phone usage in the classroom. Instructors reserve the right to dismiss the student from class due to misconduct and no refund shall be given.
I hereby expressly wave and discharge all claims and agree to indemnify, defend and hold harmless Alyssa’s School of Performing Arts from any injuries incurred during classes, rehearsals, or performances. I also understand that the registration, tuition, and other applicable fees must be paid and that I will be held responsible for all fees if I withdraw from participation after registration, until a written notice of withdrawal is received, as stated in Alyssa’s School of Performing Arts policy written above.
I acknowledge that I have read the above policies of Alyssa’s School of Performing Arts business. By my signature I acknowledge I read and agree to all Alyssa’s School of Performing Arts policies and dance program requirements.
I understand that a 2-weeks written notice before the beginning of the next month is required to terminate billing and I am responsible for payment whether or not my student attends classes until I notify Alyssa’s School of Performing Arts in writing to drop my student from class(es).
Should I dispute a charge through my financial institution this will constitute a breach of contract possibly resulting in, but not limited to, penalties, additional fees, collection, legal action, and/or termination of any and/or all current and future services. I fully understand and agree with the above statements.
Alyssa’s School of Performing Arts Liability Waiver Form
I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Alyssa’s School of Performing Arts. I/we agree to release and hold harmless Alyssa’s School of Performing Arts including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Alyssa’s School of Performing Arts liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Alyssa’s School of Performing Arts. I understand that Alyssa’s School of Performing Arts is a licensed and insured business. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Owner, Director, Instructor or Staff member as soon as possible.
I acknowledge that I have read this form in its entirety, and I understand the activity in which I will be engaging in. I accept the risks and discomfort that may come with exercise. I understand that this activity is completely voluntary and of your own free will and that I am responsible for my own actions in this facility. Having had the opportunity to ask questions and have them answered to my satisfaction, I consent to be a participant in classes with/at Alyssa’s School of Performing Arts.
