STAGE LEFT DANCE STUDIO
Impact Studios Dance and Fitness LLC · Middletown, Connecticut
ENROLLMENT AGREEMENT — 2026–2027 SEASON
Classes begin Monday, September 14, 2026
This Agreement is between Impact Studios Dance and Fitness LLC, doing business as Stage Left Dance Studio (the “Studio”), and the parent or guardian on this account (“you”), for the dancer or dancers enrolled on it.
TUITION AND PAYMENT
1. Season and tuition. The season runs from September 14, 2026 through the spring recital. Tuition is a season fee divided into nine installments — it is not a per-class charge, and it is not reduced for missed classes, holidays, or months with fewer class weeks. A dancer who enrolls mid-season pays a prorated first installment.
2. Monthly rates. Petite (3–5), Beginner (6–8), and Junior (8–11): $80 for one class per week, $110 for two, $135 for three, $160 for four or more. Teen (12–14) and Senior (15–18): $105 for one, $135 for two, $165 for three, $195 for four or more. Company, unlimited classes: Petite $130, Junior $195, Teen $200, Senior $215, Teen/Senior Elite $250.
3. Automatic payment. You authorize the Studio to charge the card or bank account on file as follows: half your monthly rate on September 14, the full monthly rate on the 1st of October, November, December, February, March, April and May, and on January 4 after winter break — nine installments in all. Autopay is our standard method and is required to hold a spot unless we agree in writing otherwise. You may cancel the authorization with written notice three business days before a charge, but cancelling does not withdraw your dancer, end enrollment, or reduce tuition — the full amount stays due on its date by another method we accept, with the late fee in Section 5 applying. You will receive a copy of this authorization.
4. Additional fees. Registration $45 per dancer, due at enrollment and non-refundable; families who registered in August 2026 owe no further registration fee. Competition costume $80 per routine, billed October 1–7. Recreational costume $70 per routine, billed February 1–7. Recital fee $75 and optional recital video $40, billed March 1–7. Competition entry fees and conventions are billed separately at cost. Costume and recital fees are non-refundable once ordered, because the Studio pays vendors in advance.
5. Late and declined payments. Each installment has a seven-day window; a $25 late fee applies to tuition not received by the end of it. If a payment is declined, you will be asked to update your payment method within seven days. If you dispute a charge you authorized here, you remain responsible for the amount plus any fee the processor charges the Studio. A dancer more than thirty days past due may be held from classes, costume orders, and recital until the balance is current. If you are facing a hardship, talk to us before a payment is missed — we would rather find a solution than lose a dancer.
6. Withdrawal. To withdraw, give written notice by the 15th of the month before you intend to stop. Tuition is charged through the end of that notice month. Registration, costume, and recital fees already paid are not refunded, and costumes already ordered remain payable.
CLASSES AND PARTICIPATION
7. Requirements. Company dancers take ballet and at least two recreational classes, as has been the case since the Studio opened. Placement in classes, companies, and routines is at the Studio’s discretion, based on level, attendance, and readiness.
8. Attendance. Attendance matters most in spring, when routines are set. A dancer with repeated absences may be removed from a routine or moved so the group can move forward. Missed classes are not credited, but may be made up in another class at their level with our approval.
9. Studio changes and closings. You may switch classes during the first two weeks of the season at no charge, subject to space. The Studio may change class times, combine classes, or change instructors based on enrollment and staffing, with as much notice as possible. Classes missed for weather or emergency closings will be made up or covered in another class where practical.
10. Pickup and supervision. Dancers under 12 must be walked in and picked up by an adult listed on this account, and changes must be given to us in writing. The Studio is not responsible for dancers before or after their scheduled class time, or for personal property left at the studio.
11. Medical care. You authorize the Studio to seek emergency medical care for your dancer if you cannot be reached, and you are responsible for its cost. The medical and allergy information on this account is accurate, and you will tell us in writing if it changes.
12. General. You agree to receive Studio communications by email and text at the contact information on this account. Connecticut law governs this Agreement, which is our entire agreement about enrollment and replaces any prior understanding, including any with the previous owner. Changes must be in writing, and if any part is unenforceable the rest stays in effect. A separate liability waiver must be accepted before your dancer participates in any class, audition, rehearsal, or event.
STUDIO POLICIES
• Dress code: dancers arrive in class attire with hair secured. No jewelry, gum, or street shoes on the floor.
• Arrive on time. A dancer more than fifteen minutes late may be asked to observe rather than dance, for their own safety.
• Keep sick dancers home. A dancer with a fever, active illness, or contagious condition should stay home until well.
• Parents watch from the lobby, not the studio, except on designated observation weeks.
• Dress rehearsal is required for recital participation. A dancer who misses it may not perform.
• Bring concerns to the owner directly, not to teachers between classes or to other families. Every concern gets a response.
• Treat teachers, dancers, and families with respect. Conduct that disrupts classes or the studio community may end enrollment.
PHOTOS AND VIDEO
The Studio photographs and films classes, recitals, and events, and uses those images to promote the Studio on its website, social media, and printed materials. If you do NOT want your dancer’s image used, email impactstudiosllcdance@gmail.com and we will note it on your account.
