I understand that I am registering my dancer for a specific summer camp, 3-week session, or intensive. Full payment is due at registration unless otherwise specified. All summer tuition and fees are non-refundable. Processing fees apply to credit/debit and ACH transactions. I understand that missed classes, schedule conflicts, or withdrawals do not qualify for refunds.

☐ Three-Week Session Makeup Policy
I understand that if my dancer misses a class during a 3-week session, I must email the studio to request a makeup. Cornerstone Ballet will do its best to accommodate makeup requests; however, makeup classes are not guaranteed due to limited class space and scheduling constraints.

☐ Camp & Intensive Attendance Policy
I understand that due to the condensed and cumulative nature of weeklong camps and multi-week intensives, makeup classes are not available for missed days.

☐ Participation Waiver & Assumption of Risk
I understand that dance classes, camps, and intensives include physical activity such as stretching, barre work, choreography, strength training, and related movement activities that carry some risk of injury. I agree to hold harmless Cornerstone Ballet and its staff from liability for injury occurring on the premises or at any studio event. I understand that Cornerstone Ballet is not responsible for lost or stolen personal property.

☐ Media & Appearance Release
I grant Cornerstone Ballet permission to photograph and/or video my child during classes, camps, intensives, and events and to use my child’s likeness, image, and appearance for promotional and advertising purposes without limitation.
