1. Financial Agreement -
Please note you are registering your dancer for a 10-month training program beginning in August and ending in May. A written 30 Day notice is required for withdrawal and must be sent to cornerstoneballetsc@gmail.com. The last day to withdraw from classes for the season is February 1st.
*Does not apply to Summer 3-week sessions, Camps, or Intensive registration. Full payment for summer programs is due at registration unless otherwise specified. All summer tuition and fees are non-refundable. I understand that missed classes, schedule conflicts, or withdrawals do not qualify for refunds.
Tuition can be paid by cash, check or debit/credit card. All accounts choosing to pay tuition in monthly installments are required to provide and keep a valid checking account or credit/debit card on file. All accounts will be drafted from the preferred payment method on file for the corresponding installment amount on the first of each month. If you would like to pay by an alternate method (Cash or Check) this can be done up to 2 business days before the scheduled drafting date at the front desk. Payments can be made through your online account with Dance Studio Pro with a credit or debit card. There will be a 3% + $0.35 processing fee for all credit/debit card transactions and a 2% + $0.35 fee per ACH checking account transaction. Please be sure to contact us immediately if there are any changes in your preferred payment method. All tuition and fees are non refundable. A late fee of $15 will be charged after the 1st of the month to any account with a balance. Please note that any accounts with unpaid balances will be subject to collections with all associated fees.
*If your family experiences unexpected financial hardship please reach out to us before withdrawing your dancer. Cornerstone Ballet Performing Arts is a 501(c)3 non profit organization that has a scholarship program available to help provide students with the opportunity to study classical ballet.
2. Agreement for Participation - I understand that dance classes may include, without limitation: dancing with props, stretching, barre work, across the floor combinations, dance routines in the center, and other related activities. I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury. I agree to hold harmless Cornerstone Ballet and its staff from liability from any injury occurring on the premises of Cornerstone Ballet or at any Cornerstone Ballet event. Cornerstone Ballet is not responsible for personal property. I agree to be responsible for reading studio correspondence and respecting deadlines. I understand that in extreme circumstances, classes can be moved fully or partially to a virtual live class platform at any point in time which Cornerstone Ballet believes meeting in person at the studio is not in the best interest of the health and safety of its students and staff. I hereby acknowledge that I have read the statements above and agree to participate accordingly.
3. Appearance Clause - I understand that Cornerstone Ballet produces promotional material about their programs. I understand that my son/daughter may be included in video tape or photography taken during class and/or events, and I hereby grant Cornerstone Ballet, its successor, assignees, licenses, sponsors, any television networks, and all other commercial exhibitors the exclusive rights to photograph and/or video tape my son/daughter and further to utilize my son/daughter’s name, face, likeness, voice, and appearance as part of the event/class/etc., and in advertising and promotion of the event/class/etc. without reservation or limitation. In granting this license, I understand that Cornerstone Ballet is under no obligation to exercise any of its rights, licenses, and/or privileges herein granted.
4. Attendance & Makeup Class Policy - Students who miss a class may be eligible to attend a makeup class within 30 days of the absence. Makeup classes will be scheduled in a class of the same level—or the closest appropriate level—based on availability and at the discretion of the studio staff. It is the sole responsibility of the dancer's parent or guardian to arrange any desired makeup classes in a timely manner. All makeup classes must be completed within 30 days and in the same class season in which the absence occurred. A maximum of four (4) makeup classes are allowed per season.
