I understand and agree with the above payment terms and the policies of Mid-County Dance.

I acknowledge that once an initial costume payment is made, the account holder assumes responsibility for the full cost of the costume.

I acknowledge that I am financially responsible for any damage to Studio property caused by my child, including any costs associated with repair or replacement.

I acknowledge that tuition & costume payments are non refundable.

If withdrawing from classes, you must give the office a 30 day notice in writing. Until letter is received, student is considered enrolled & tuition must be paid
