I have chosen to have my child(ren) participate in dance instruction given by Acro Academy.
I acknowledge that I understand the nature of the activities of my child will be participating in and that my child is in the proper physical condition and capable of participating in the related activities, understanding that Acro Academy is not in any way responsible for making such a determination.
In consideration of my child(s) enrollment in any dance instruction program, I understand and agree on behalf of myself and my child, to release, hold harmless, and discharge Acro Academy from all claims, costs, liabilities, expenses, or judgments, including attorneys (s) fees and court costs for any occurrences in connection with any dance instruction. I assume all risks to my child in connection with any instruction and further release Acro Academy and its owners and employees from liability for injury sustained by my child while he or she is enrolled in any dance instruction program, including all risks reasonably connected with such activity whether foreseen or unforeseen.
I understand that Acro Academy is not responsible for my child or any other children under their supervision who are left unsupervised in the common areas and areas surrounding the dance studio and that Acro Academy will only be supervising my child when they are participating in scheduled dance activities, programs, or instruction. I understand that Acro Academy is not responsible for personal property that is lost, damaged or stolen while my child is at Acro Academy.
I acknowledge and agree that it is my responsibility to maintain my own accident and health insurance coverage that provides adequate coverage for myself and my child participating in Acro Academy activities and that Acro Academy does not provide accidental or health insurance for those participating in its instruction, activities, or programs.
I authorize and agree that Acro Academy may take and use photographs, videos or likenesses of myself, or my child as needed for its record keeping, advertising, and or public relations projects and that I have no rights to the same and will not be compensated for the same.
My signature is proof of my intention to execute and complete and unconditional waiver and release of all liability pursuant to the terms herein, and agreement as to all terms and conditions contained above. I am of lawful age and competent to sign this affirmation.
I understand that there is a risk of contracting COVID while dancing at Acro Academy. I will not knowingly attend a class at Acro Academy if I or my child present any symptoms of illness. I understand that my child will be sent home if he or she has any illness symptoms present.


Agreement of classroom protocol:
I understand that the accepted practice and methodology of professional dance training entails, hands on and other forms of physical contact with instructors and other students, and that it is necessary for a student to learn at an appropriate pace. I understand that if there is currently, or ever would be, any reason why my child should not be touched or touched in a specific way, that it is my responsibility to inform EACH instructor as to what is acceptable, and to discuss and related physical or mental issues with them.

I HAVE FULLY INFORMED MYSELF AS TO THE CONTENTS OF THIS RELASE AND HAVE READ THE SAME PRIOR

Parents and guardians have the right to formally request that their child not be included in any promotional photos, or videos taken during classes, rehearsals, or studio events. To opt out, a signed photo release Opt-Out form must be submitted to our administrative team.

Please note :

1. Opting out means we will do our best to exclude your child from any intentionally captured promotional content

2. Group photos/videos may sometimes include students incidentally. While we make every effort to respect opt - out requests, complete exclusion in all contexts may not be guaranteed unless your child is removed from the filming activity.

Surveillance Policy:
For the safety and security of all students, staff and visitors;

1. surveillance cameras are running at all times inside Acro Academys common areas and instructional areas.

2. These cameras are strictly for internal security and operational purposes only.

3. Surveillance footage will not be released or provided to any individual.

4. No footage may be duplicated, shared or distributed under any circumstance unless required by legal authorities or for the direct safety and protection of individuals involved.

By enrolling in or attending Acro Academy, you acknowledge and accept the presence of surveillance for safety purposes and understand your right to request your childs exclusion from promotional materials.

By engaging in email, phone or message communication with Acro Academy, you acknowledge and agree that your correspondence may be accessed, reviewed and responded to by two authorized representatives of Acro Academy: Ashley Gauthier and Jamie Sims. These individuals are jointly responsible for handling all communications, accounts and personal files on behalf of Acro Academy.

All communications received by Acro Academy, including but not limited to emails, phone calls and text messages are treated as PRIVATE and CONFIDENTIAL. No communication will be shared with any third party outside of the organization without your explicit consent.

Disclosure Prohibited.
Any unauthorized distribution, publication or sharing of
1.Emails, messages or other correspondence to or from Acro Academy - including posting on social media, public forums, or any other platform - without our written permission constitutes a breach of this agreement.

2.Photos, videos or information involving any individual other than your child.

3.All individuals who enter Acro Academy- whether as students, staff , parents, contractors or guest - are entitles to complete privacy.

However, as a client of Acro Academy, you understand and acknowledge that;

1.The studio is a shared environment, and complete anonymity cannot always be granted.

2. Students may attend classes at the same time, and friendships or prior relationships between families may naturally result in some awareness of who is participating and or present.

3. While Acro Academy upholds the highest standard of privacy and confidentiality, it is not responsible for casual incidental recognition among clients who already know one another outside of the studio.

Such breaches may result in legal action or financial penalties as deemed appropriate by Acro Academy.

By continuing to communicate with Acro Academy, you consent to this policy and acknowledge your understanding and agreement to uphold the privacy and confidentiality of all correspondence.

I understand that Acro Academy has a Refund Policy in place.

1. Any non competitive program such as ; 8 week programs, 4 week programs, camps, or pop up class payments are to be paid in full before the date of the first class. A refund will not be given after the second scheduled class. A 50% refund will be given after the first scheduled class.

2. Workshops are non refundable

3. Competitive Costumes, and competition fees are non refundable

3. Competitive programs are non refundable after the month of October 1st of the participating year. All 10 month payments must be paid in full regardless of childs attendance throughout the 10 month program of the participating year of September 1 - June 1.
