I, on behalf of myself, my heirs, executors, agents, assigns and representatives, hereby indemnify, release and forever hold harmless Dance Star Academy of Performing Arts (DSAPA) as well as its Director(s), employees and instructors from any and all claims of liability arising from any accident, personal injury, death, or property loss or damage sustained by my child/my self/the minor child for whom I am a legal guardian, while that person is participating in activities connected with Dance Star Academy of Performing Arts, including classes, rehearsals, performances, or other activities. I understand that dance activities have inherent risks or injury, and, being fully aware of all risks, I consent to have my child/my self/the minor child of whom I am a legal guardian, participate in the program and activities offered by DSAPA, and I accept full responsibility for providing adequate health and accident insurance coverage for the protection of my child/my self/the child of whom I am a legal guardian who participate in these activities. I am a legal guardian. By signing this statement, I declare that the aforesaid participant is in good health, with no physical conditions that might prevent his/her/my participation in strenuous and rigorous dance activities.
PARKING POLICY
I understand I am to use the DSAPA parking lot for ALL drop offs, parking, and pick-up.
I understand I am NOT allowed to drop off on Main Street in front of the back gates.
I understand I am NOT allowed to park on Main street.
I understand I am NOT allowed to park in the Mortuary parking lot until after 5:30pm.
HAIR & DRESS CODE
I understand the appropriate hair and dress code at DSAPA as outlined on our website at: https://www.dsapa.com/dress-code
CLASSROOM POLICY
I understand that all DSAPA staff members reserve the right to dismiss anyone disrupting a class.
MAKE-UP CLASSES
If my child has an absence and needs to schedule a makeup class, I understand makeup classes MUST be scheduled with a DSAPA office staff member within 30 days of the absence.
DROPPING POLICY
Dance tuition is charged on the 1st of the month.
I understand that if my child will be dropping a class, I MUST give notice to the Director by the 15th of the month through email: director@dsapa.com. Providing notice by the 15th will avoid a next month tuition charge.
If I fail to give notice on or before the 15th of the month, I will be charged full tuition for the next month. There are no refunds for a class that is dropped after the month has started.
RECITAL/ SHOWCASE POLICY
I understand that all DSAPA Recitals and Showcases are MANDATORY for every dancer.
I understand the only classes that do NOT participate in recital are the following: Leaps & Turns, Tumbling, Stretch & Strength, Combo & Improv, or any other technique classes.
I understand that my child must participate in the recital or showcase for all performance classes. I understand that the recitals are mandatory. Classes such as tumbling, stretch and strength, leaps and turns, and combo and improv classes will not have a recital routine.
I understand I will be charged a $25 Recital Fee per dancer.
I understand that DSAPA normally hosts two recitals per year.
I understand that each class will be charged a costume fee ranging from $65-$75
REGISTRATION FEE
I understand I will be charged a $25 registration fee for my child.
I understand I will be charged a $10 registration fee for each additional child in the same family.
TUITION POLICY
I understand monthly tuition is due ON OR BEFORE the 1st day of the month. After the 5th of the month, I understand I will be charged a $15.00 late fee.
I understand I MUST have a credit card on file. You can avoid a charge to your card by paying cash in advance. Please contact the director to schedule a cash payment.
I understand there are NO reduced rates or refunds for any illness, vacation, or missed classes. Some months have a 5th class and other months only have 3 classes due to holiday closure. Tuition rates will not be adjusted in either circumstance.
