I understand that I assume full responsibility for the risk of bodily injury and death, whether due to the negligence or gross negligence of the Released Parties or otherwise. I voluntarily accept those risks.
 
I understand that it is my sole and exclusive responsibility to purchase health and/or accident insurance and agree to be liable for and pay all costs and expenses incurred with any medical and/or hospital services rendered to the child and/or children as a result of any activities associated with Elevation Dance Refinery

Registration fee of $35 per family per year is due. Classes are annual programs with the freedom to drop in/out at anytime throughout the year. Tuition is an annual fee broken into 12 monthly payments plus an annual registration fee based on the school year (August-May). Registration can be done online. Enrollment is perpetual. Once a student is enrolled they are enrolled until a parent contacts us to officially drop from the program. All families must keep a valid credit card on file..if you wish to not please send us an email to make adjustments. Parents/Guardians can drop a student from the program by submitting a drop request in writing (email is just fine). A student is officially dropped once a confirmation email from us has been received by the billing contact on file. Send drop emails to elevationdanceindy@gmail.com. Tuition is charged on the 15th of the month to the credit card on file. Drop requests must be sent by the 14th of the month to avoid credit card on file being charged. Makeups are allowed and must be completed within 30 days of the missed class. Students must be enrolled at the time of makeup class redemption. Makeups can be scheduled via email. Once a credit card has been charged there are no refunds or credits for any reason. If the card on file is declined the billing contact has 7 days to update the card on file. Repeated declined credit cards can result in removal from the program. Siblings or students who register for multiple classes are eligible for a discounts. There are no credits or refunds for missed classes. Tuition holds a place in a class and cannot be adjusted for attendance. All credits are good for 90 days only.

I hereby authorize Elevation Dance Refinery, Mahogany Contemporary Dance Academy and Meraki Dance Company, its representatives, successors and assigns and all persons acting with its permission to take, copyright, use and publish photographs and/or video of my dancer. I hereby release, discharge and agree to hold harmless Elevation Dance Refinery/Meraki Dance Company/Mahogany Contemporary Dance Academy and all persons acting with its permission or upon its authority from any liability for or arising out of taking, copyrighting, using and publishing photographs and/or video of my dancer.
