WAIVER/RELEASE OF LIABILITY
The practice of dance involves the risk of physical injury (with bruises being the most likely injury and broken bones or other more serious physical injuries also being possible.) Understanding this I declare:
1. That I am willing to accept responsibility for such an eventuality, and;
2. That I have adequate medical insurance for such an eventuality, and;
3. That I will not hold any club, member, instructor, or the owners or operators of any facility in which I might practice liable for any injury that I might
sustain while practicing dance.
Further, I understand the physical demand of this activity and the practice required for its development and mastery. As a consideration for my own safety and enjoyment, as well as that of other students, I commit to dedicate necessary practice of the instructions and techniques given to me in class.
PHOTO/MEDIA RELEASE
I/we agree to allow Monarch Academy of Dance to photograph or videotape me or my child at any Monarch Academy of Dance event related to the use of any and all spaces used by Monarch Academy of Dance for the use of advertising and literature for Monarch Academy of Dance. This is included but not limited to in the studio, recital, competitions, workshops, conventions, and fundraisers. No compensation will be allotted for the use of these photographs or videotapes.
STUDIO POLICIES
SPRING CONCERT:
Participation in our end of year concerts and rehearsals are mandatory. Please note that there are additional costs associated with taking part in these extra events that will include a $75 per-dance costume fee which is charged to your account on 10/1. Other optional costs include recital tickets, flowers, program messages, and keepsake gifts. Details on these events will be sent to all families closer to the time.
TUITION PAYMENTS:
ACH (direct debit from checking account) is our preferred method of transaction. Save 5% when you sign up for ACH at front desk!
New students will enroll in a program of a specific length. After an initial down payment (enrollment fee $99), the remaining cost of the program will be split into installments to be paid monthly throughout the program. There are no refunds of program down payments, installment payments, or tuition payments made in full. I understand that this authorization will remain in effect until I cancel it in writing, and I agree to notify Monarch Academy of Dance in writing of any changes in my account information or termination of this authorization at least one month prior to the next billing date.
MISSED CLASSES AND MAKE-UPS:
Attendance is crucial in the progress of the class and the dancer. Students need to consider their other activities/work schedules before making a commitment to dance and it is your responsibility to attend your regularly scheduled class. Monarch Academy of dance does allow students to make up missed classes in a level at or below their current placement. Students more than 10 minutes late to class will be asked to sit and watch for their safety. Missing the first 10 minutes of class means they have missed the warm up portion of class, and therefore, participation is not possible without the risk of injury. There will be no pro-rated tuition OR refunds given for missed classes.
SCHEDULED CLOSURES:
The studio will be closed for many federal holidays and each winter between the Christmas and New Year Holiday. Your program tuition includes these closures.
WINTER WEATHER CLOSURES:
In the event that a decision is made to close Monarch Academy of Dance due to inclement weather students will be notified by email and text; as well as announcements posted on our Facebook Page and BAND app. Make-up class dates and times will be released on an as needed basis in this event.
ADDITIONAL COSTS:
Students can expect to pay participation and costume fees for performances. These will vary depending on the performance and the program in which the student is currently enrolled.
UNIFORMS:
With the exception of adult classes, all Monarch Academy of Dance students are expected to bring and wear a studio uniform to class. The use of a uniform increases the level of focus, organization, and professionalism in class. Loaner shoes and leotards will NOT be provided. Please DO NOT add a tutu or skirt to the leotard if the dancer's uniform does not include one. NO street shoes are to be worn in the studio at any time. Only dance approved shoes are allowed.
FOOD/BEVERAGE POLICY:
Only water is permitted inside the studio spaces. Sugary drinks such as juice, sodas, and sports drinks are not permitted. Dancers should always have a healthy snack and water packed for their evening or day of classes and rehearsals. Lunch may be packed in a cooler if the dancer has a long day. Fridge space is limited to employees only.
TEACHER LOUNGE:
At Monarch, there is a teacher lounge that is off limits to students. Please respect this as a private space for employees to have their breaks. If a dancer should need to access something in the lounge, please knock or ask permission to enter.
