I understand and agree that participation in any dance class, tumbling class, workshops, performances, etc. poses a possibility for physical injury. I voluntarily agree, therefore, to assume all risk and responsibility for any such injury or accident which may occurs to me or my child during any Susan Ina Academy of Performing Arts, LLC, classes, workshops, performances or activities. I also understand that this is a hands-on class instruction studio. I understand that I should be aware of my physical limitations, as well as those of child, and agree not to exceed them. I certify that I am the parent/legal guardian of the registering child and have the right to waive these rights.

I understand that the studio pays careful attention to sterilization and disinfection practices. There is still a chance that you or your child could potentially be exposed to an illness on the Susan Ina Academy of Performing Arts LLC property. Please know that participation in classes, workshops, performances, or activities hosted by Susan Ina Academy of Performing Arts are at your own risk.

I understand that my child/children may be photographed or recorded by the Susan Ina Academy of Performing Arts, LLC for advertising purposes on social media.
