PARTICIPATION WAIVER, RELEASE OF LIABILITY, AND MEDIA RELEASE AGREEMENT
1. Voluntary Participation & Assumption of Risk: I understand that dance training and related physical activities involve inherent risks of physical injury. By enrolling my child, I voluntarily assume all risks associated with such participation. I certify that my child is physically fit and capable of participating safely in the programs offered by Arco City Ballet.
2. Release of Liability & Indemnification: Liability: In consideration of my child’s enrollment, I hereby release, waive, discharge, and covenant not to sue Arco City Ballet, its directors, officers, employees, volunteers, and independent contractors from any and all liability, claims, demands, or causes of action arising out of any physical injury, accident, or illness that may occur during the course of participation in classes, rehearsals, performances, or events.
3. Media & Promotional Release: I hereby grant Arco City Ballet irrevocable permission and the absolute right to capture, use, publish, and distribute photographic images, video recordings, or audio recordings of my child taken during studio activities for educational, promotional, marketing, and social media purposes, without compensation or further notice.
4. Acknowledgment of Terms: By checking the box/signing below, I acknowledge that I have read this document in its entirety, fully understand its terms, understand that I am giving up substantial legal rights, and agree to it freely and voluntarily.
