I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Naatyaveda Dance Academy LLC.

I/we agree to release and hold harmless Naatyaveda Dance Academy LLC including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Naatyaveda Dance Academy LLC liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Naatyaveda Dance Academy LLC.

I understand that Naatyaveda Dance Academy LLC is a licensed and insured business. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Owner, Director, Instructor or Staff member as soon as possible.

I consent that the photographs and/or motion picture/video film the Naatyaveda Dance Academy LLC classes, rehearsals, and/or performances may be used for promotional and/or marketing purposes, including television, website, social media or other electronic purposes. Furthermore, I hereby consent such photographs, video recordings, film and all other visual mediums may be used free and clear of any claim whatsoever on my part.

I understand that participation in dance classes, rehearsals, performances, workshops, and other activities at Naatyaveda Dance Academy LLC involves physical movement and may include the risk of injury.

I voluntarily permit myself/my child to participate in these activities and acknowledge and accept the risks normally associated with dance instruction and physical activity.

I agree to release and hold harmless Naatyaveda Dance Academy LLC, its owner, instructors, employees, assistants, volunteers, and representatives from claims or liability arising from ordinary risks associated with participation in academy activities, except where prohibited by law.

I understand that students are expected to follow all safety instructions provided by the instructor and to inform the academy of any injury, medical condition, physical limitation, or other circumstance that may affect safe participation.

By accepting this waiver, I confirm that I have read and understood the above information and voluntarily consent to participation.

I certify that I have informed Naatyaveda Dance Academy LLC of any relevant medical condition, allergy, injury, medication, physical limitation, or other health information that may affect myself/my child’s participation in dance activities.

In the event of an illness, injury, or medical emergency, I authorize Naatyaveda Dance Academy LLC, its instructors, employees, or authorized representatives to contact emergency medical services when they reasonably believe immediate medical attention is necessary.

If I cannot be reached promptly, I authorize appropriate emergency medical treatment to be provided to myself/my child as deemed necessary by qualified medical personnel.

I understand that Naatyaveda Dance Academy will make reasonable efforts to contact the parent/guardian or emergency contact as soon as possible.

I understand that I am responsible for any medical expenses resulting from emergency care, transportation, or treatment.

By accepting this authorization, I confirm that the emergency contact and medical information provided to the academy is accurate and current.

Naatyaveda Dance Academy LLC may occasionally take photographs or video recordings during classes, rehearsals, performances, recitals, workshops, and academy events.

By accepting this waiver, I give Naatyaveda Dance Academy permission to photograph and/or record myself/my child and to use these images or recordings for academy-related purposes, including:

•⁠ ⁠Social media
•⁠ ⁠Website and promotional materials
•⁠ ⁠Flyers, brochures, advertisements, and newsletters
•⁠ ⁠Performance or recital promotions
•⁠ ⁠Educational or archival purposes

I understand that these photographs or recordings may be published without additional compensation.

The academy will not intentionally publish sensitive personal information such as a student’s home address, phone number, or other private information.

If I do not wish for myself/my child to be photographed or recorded, I understand that I must notify Naatyaveda Dance Academy in writing.

By enrolling at Naatyaveda Dance Academy LLC, I acknowledge that I have received, reviewed, and agree to follow the academy’s current rules, policies, and expectations.

I understand and agree that:

•⁠ ⁠Tuition is due on the 1st of each month.
•⁠ ⁠Payments received after the applicable grace period may be subject to a late fee.
•⁠ ⁠Registration fees are non-refundable.
•⁠ ⁠Tuition covers the scheduled lessons for the semester and is not calculated based solely on the number of classes occurring in an individual calendar month.
•⁠ ⁠Missed classes due to a student’s absence are not refundable or credited.
•⁠ ⁠Make-up classes for student absences are not guaranteed.
•⁠ ⁠If the academy cancels a regularly scheduled class, an appropriate make-up class will be arranged when necessary.
•⁠ ⁠Students are expected to arrive on time, follow the academy dress code, bring required materials, and behave respectfully toward instructors and fellow students.
•⁠ ⁠Regular attendance and practice are important for student progress.
•⁠ ⁠Participation in community performances may be optional; however, once a student commits to a performance, attendance at required rehearsals is expected.
•⁠ ⁠Participation in the academy’s annual recital/program may be required according to the academy’s current recital policy.
•⁠ ⁠Additional expenses associated with performances, recitals, costumes, choreography, workshops, competitions, or special programs are separate from regular tuition unless otherwise stated.

I understand that academy policies may be updated when necessary and that families will be informed of significant changes.

By accepting this agreement, I acknowledge that I am responsible for following the current Naatyaveda Dance Academy policies.

Enrollment at Naatyaveda Dance Academy LLC continues until the academy receives proper notice of withdrawal.

A minimum of one month’s written notice is required before withdrawing a student from classes.

Verbal notification to an instructor or absence from class does not constitute official withdrawal. Withdrawal notice must be submitted in writing to Naatyaveda Dance Academy.

Tuition remains due during the required notice period, regardless of whether the student attends classes.

Registration fees and previously paid tuition or other applicable fees are non-refundable unless otherwise required by law or specifically approved by the academy.

Failure to attend classes does not automatically cancel enrollment or eliminate outstanding tuition obligations.

By accepting this policy, I acknowledge that I have read and understand the withdrawal requirements.
