I understand I am signing up for an account with Melina's Dance Company and may receive emails and/or texts regarding my child's enrollment or notifications from MDC. MDC will not share or sell personal information.

Melina’s Dance Company (MDC) Participation Waiver and Release of Liability

The undersigned acknowledges and understands that participation in dance and movement classes, programs, trainings, rehearsals, and performances at Melina’s Dance Company (MDC) involves inherent risks, including but not limited to the risk of physical injury.

In consideration of being permitted to participate in MDC activities, the undersigned, on behalf of themselves and any participating child(ren), agrees to the following terms:

Assumption of Risk
The undersigned voluntarily assumes all risks related to participation, including illness, bodily injury, death, or property damage, whether occurring on MDC premises or at off-site locations.

Release of Liability
The undersigned, on behalf of themselves and any participating child(ren), hereby releases, waives, and discharges MDC, its owners, instructors, staff, and volunteers from any and all claims, demands, actions, or liabilities for any injury, illness (including but not limited to COVID-19 or other communicable diseases), death, or property loss/damage that may arise from participation, whether caused by negligence or otherwise.

Indemnification
The undersigned agrees to indemnify, defend, and hold harmless MDC, its owners, instructors, staff, and volunteers from any and all claims, liabilities, damages, or costs (including attorney’s fees) resulting from participation by the undersigned or their participating child(ren).

Medical Acknowledgment
The undersigned affirms that neither they nor their participating child(ren) will attend or participate in MDC activities if experiencing symptoms of illness or if under a medical directive to isolate. The undersigned agrees to notify MDC promptly of any known exposure to communicable diseases that may affect participation.

Insurance Acknowledgment
The undersigned understands that MDC does not provide medical, health, or accident insurance for participants, and that it is the participant’s responsibility to carry personal insurance coverage.

I ACKNOWLEDGE THAT I HAVE READ, UNDERSTAND, AND VOLUNTARILY ACCEPT THE TERMS OF THIS WAIVER AND RELEASE.

Melina’s Dance Company (MDC) Photo and Video Release

I, the undersigned, hereby grant permission to Melina’s Dance Company (MDC) to capture and use photographs and/or videos of myself and/or my minor child(ren) for promotional, marketing, and educational purposes. This may include, but is not limited to, use in printed materials, social media, advertisements, videos, the MDC website, and other online platforms.

I understand and agree to the following:

I am the parent or legal guardian of the participating child(ren) registered at MDC and have full authority to grant this permission.

I waive any expectation of confidentiality for myself and/or my child(ren) regarding the use of such media.

I understand that no financial compensation will be provided for the use of these images or videos.

I acknowledge that participation in MDC media content does not confer any rights of ownership or entitlement to myself or my child(ren).

I release and hold harmless Melina’s Dance Company, its owners, instructors, staff, volunteers, and contractors from any claims or liabilities arising out of or related to the use of these photographs and/or videos.

Melina’s Dance Company (MDC) Tuition Policy

Full monthly tuition is due by the 5th of each month.

The final tuition payment of the season is due June 1st.

Account balances must be paid in full by June 10th for dancers to participate in the June recital.

A $15 late fee will be applied to accounts with an unpaid balance as of the 10th of the month.

MDC’s tuition schedule is based on a full 10-month dance season (September – June) and includes a minimum of 35 class meetings per program. Shorter months are balanced by longer months to meet this schedule.

Please note:

Tuition is not prorated for student absences. Holidays or studio closures due to weather or events have been built into the dance season.

Dancers are encouraged to attend makeup classes for any missed sessions.

Payment Options:

Monthly tuition can be paid via automatic withdrawal using a Visa, MasterCard, Discover, or American Express, or via ACH; convenience fees apply to all transactions.

We also accept cash, checks, or money orders for tuition payments at the studio. Please include your child’s name on the memo line of all checks.

Melina’s Dance Company (MDC) Withdrawal Policy

To withdraw a student from MDC classes:

Parents/guardians must notify the studio in writing.

Not attending class does not constitute a withdrawal; tuition will continue to be charged until a formal withdrawal is completed.

Notice Requirements:

A 14-day written notice is required. Tuition remains billable during this notice period.

To avoid being charged for the next month, withdrawal notices must be submitted by the 15th of the current month.

Important Notes:

Membership fees, tuition, costume fees, and performance fees are nonrefundable and will not be prorated.

If costumes have already been ordered at the time of withdrawal, families are responsible for the full costume payment. Costumes may be picked up after May 15th.
